蝎竭中药膏外用联合腔内激光闭塞术治疗下肢静脉曲张性溃疡66例临床观察

【摘要】目的观察蝎竭药膏外用结合腔内激光闭塞术治疗下肢静脉曲张性溃疡临床疗效。方法蝎竭中药膏结合激光腔内闭塞术治疗下肢大隐静脉曲张继发溃疡66 例，患者入院后先行蝎竭中药膏外用及抗感染治疗，溃疡愈合后为避免溃疡复发，再行静脉激光腔内闭塞术以治本。结果66例静脉曲张均得以祛除，术后2周CT造影显示大隐静脉及浅表静脉团均闭塞，且术后1年随访无复发溃疡。结论蝎竭中药膏外用结合腔内激光闭塞术治疗下肢静脉曲张性溃疡能快速愈合伤口、祛除静脉曲张，标本兼治。

Abstract【】ObjectiveTo observe the clinical effects of Xiejie Chinese medicine ointment topical combined with intracavity laser occlusion on variceal ulcer of lower exthemities.Methods66 patients with great saphenous vein of lower limb varicose secondary ulcer were treated by Xiejie Chinese medicine ointment topical combined with intracavity laser occlusion.Patients after admission were the first in Xiejie Chinese medicine ointment for external use and anti-infection treatment.To avoid the ulcer recurrence after ulcer healing，laser cavity vein occlusion was carried on.ResultsVaricose veins of 66 patients were dispelled，CT angiography of 2 weeks after surgery showed great saphenous vein and superficial vein group were blocked,there were no postoperative ulcer recurrence at one year follow-up.ConclusionThe application of Xiejie Chinese medicine ointment topical combined with intracavity laser occlusion on variceal ulcer of lower extremities can rapid wound healing,eliminate varicose veins,treat both manifestation and root cause of disease.

下肢静脉曲张性溃疡(variceal ulcer of lowerextremities，VULE)是临床常见的周围血管病，一般好发于踝上及足靴区，中医称之为臁疮。溃疡常年不愈或愈后复发，是顽疾之一，治疗棘手。国外统计VULE的发生率和复发率分别为0.5%～3%、67%，10年以上静脉曲张病史的溃疡发生率为10%[1]。该病西医治疗采取抗生素、水凝胶、生长因子等外用，只能暂时控制溃疡面的进一步腐烂和坏死，无法有效地促进其生肌长肉，伤口难愈合。中医外治法源远流长，效果明显，传统的外用煨脓长肉中药有很好生长肉芽功效，但愈后可能复发。……
