社会心理干预对精神分裂症患者社区康复的作用

目的：探讨社会心理干预对精神分裂症患者康复的作用。方法将109例精神分裂症患者随机分为社会心理干预组56例和对照组53例。干预组接受药物治疗和社会心理干预，对照组仅接受药物治疗。分别对2组的阳性和阴性症状量表(PANSS)，社会功能缺陷筛选量表(SDSS)和依从性评分表、自知力、复发率进行比较。结果随访3、6、12个月干预组PANSS总分、各因子分、SDSS评分显著低于对照组(P<0.05)，治疗依从性、自知力好于对照组(P<0.05)，复发率明显低于对照组(P<0.05)。结论社会心理干预对精神分裂症的疗效，治疗依从性提高，社会功能改善及降低疾病的复发率有很好的作用，在我国精神分裂症社区康复工作中切实可行。

Abstract：[Objective] To explore the function of social and psychological intervention on the rehabilitation of patients with schizophrenia. [Method] 109 patients with schizophrenia were randomly divided into intervention group 56 cases and control group 53 cases. Conventional treatments were given to the patients in both groups and social intervention was applied to the patiens at the same time to intervention group. All patients were assessed with positive and negative syndrome scale (PANSS), social disability screening schedule (SDSS), insight treatment attitude questionnaire (ITAQ), the compliance scale and self-designed scale at time of enrollment, 3 months, 6 months and 1 year after enrollment. [Result] The readmission rate in one year and the scores of PANSS were lower in observation group than that in control group (P<0.01or0.05)； ITAQ score was higher than that in control (P<0.01). Between the two groups, the difference was significant(P<0.01or0.05)about medication compliance, relapse rate and employment rate. [Conclusion] Psychological and soical intervention can help patients with schizophrenia strengthen adherence of treatment, improve their social function and decrease the rate of recurrence.

社区慢性病管理，既是社区卫生工作的职责，也是慢性病治疗的一个重要途径，精神分裂症患者的治疗也不例外。社区康复对患者社会功能改善和降低复发具有十分重要的作用，但精神分裂症患者属于一类十分特殊的患病群体，其社区康复中所实施的社会心理干预，须有相应资质的人员才能完成。作为基层医疗卫生服务的龙头，县级医疗卫生单位在精神分裂症患者社区康复中负有重要使命。我院自2010年起，通过社会心理干预，开展精神分裂症患者的社区康复，取得了较好的效果。现报告如下。
