【摘要】目的：探討非布司他在老年糖尿病肾病合并高尿酸血症中的治疗效果。方法：选择110例老年糖尿病肾病合并高尿酸血症患者为研究对象，时间为2022年2月—2023年2月，将受试者根据集中抽样法进行分组，各55例，对照组患者口服别嘌呤醇片治疗，研究组患者则联合非布司他治疗，连续治疗3个月后评价两组患者的治疗总有效率、血糖水平、血尿酸及各肾功能指标变化，并评价两组患者的用药安全性。结果：研究组治疗效果94.55%优于对照组78.18%（P<0.05）；治疗后两组患者FBG、2hPG及SUA水平较治疗前均明显降低，且研究组更优（P<0.05）；治疗后两组患者24hUpro、Cys-C、BUN及Scr水平较治疗前均明显降低，且研究组更优（P<0.05）；研究组患者药物所致不良反应发生率较对照组低（P<0.05）。结论：非布司他联合应用可协同性的控制血糖及血尿酸水平，改善肾功能，疗效安全显著。

Therapeutic effect of febuxostat in elderly diabetic nephropathy complicated with hyperuricemia

【Abstract】Objective： To investigate the therapeutic effect of febuxostat in elderly diabetic nephropathy complicated with hyperuricemia. Methods： 110 elderly patients with diabetic nephropathy complicated with hyperuricemia were selected as the research subjects， and the time was from February 2022 to February 2023. The subjects were divided into groups according to the centralized sampling method， with 55 cases in each group. The patients in the control group were treated with oral allopurinol tablets， and the patients in the study group were treated with febuxostat on the basis of the control group. After 3 months of continuous treatment， the total effective rate of treatment， blood glucose level， blood uric acid and changes in renal function indexes of the two groups of patients were evaluated， and the drug safety of the two groups of patients was evaluated. Results： The treatment effect of the research group was 94.55%better than the control group 78.18%（P<0.05）. After treatment， the levels of FBG， 2hPG and SUA in the two groups were significantly lower than those before treatment， and the study group was even better（P<0.05）. After treatment， the levels of 24hUpro， Cys-C， BUN and Scr in the two groups were significantly lower than those before treatment， and the study group was even better（P<0.05）. The incidence of drug-induced adverse reactions in the study group was lower than that in the control group（P<0.05）. Conclusion： The combined application of febuxostat can synergistically control blood sugar and blood uric acid levels， improve renal function， and has significant curative effect and safety. It has positive significance for the disease control of elderly patients with diabetic nephropathy complicated with hyperuricemia.

糖尿病肾病是糖尿病最为常见的一种并发症，发生率约为20%～40%，若未及时给予对症干预，随着病情进展则可导致不可逆性肾功能损伤，最终发展为肾衰竭、尿毒症等终末期肾病，给患者的生命安全造成极大威胁[1]。高尿酸血症属于一种代谢性疾病，主要因嘌呤代谢障碍使尿酸量升高所致，常会损伤肾小管及肾小球，使肾功能减退，是慢性肾脏疾病发生的独立危险因素[2]。别嘌呤醇片是临床治疗高尿酸血症的常用药物，可抑制黄嘌呤氧化酶活性，进而使尿酸水平降低，但易损伤肾功能，单独应用效果欠佳。非布司他属于一种票另类制剂，具有较好的降尿酸效果，且不会损伤肾功能[3]。为此，该研究对110例老年糖尿病肾病合并高尿酸血症患者的临床资料进行研究，进一步分析非布司他的应用效果，现报道如下。
