【摘要】目的：分析责任制助产护理结合体位管理在初产妇分娩中的运用效果及对分娩结局的影响。方法：取2022年1月—2023年1月在我院进行生产的初产妇100例，随机分为两组各50例。对照组采用常规助产护理，观察组采用责任制助产护理结合体位管理护理，以比较助产效果。结果：与对照组相比，观察组初产妇的护理满意度更高，分娩结局较好，产程用时较短，经护理后，产妇的疼痛情况和心理状况较好（P<0.05）。结论：针对初次分娩的产妇，采取责任制助产护理结合体位管理的效果更优，产妇的分娩结局以及产程时长有明显的改善，且产妇护理后心理状况得到提升，护理优势突出，可在临床大量推广。

The application effect of responsible midwifery nursing combined with posture management in primiparous delivery and its impact on delivery outcomes

【Abstract】Objective： To analyze the application effect of responsible midwifery nursing combined with posture management in primiparous delivery and its impact on delivery outcomes. Methods： 100 primiparous women who gave birth in our hospital from January 2022 to January 2023 were randomly divided into a control group and an observation group based on the admission number， with 50 cases in each group. The control group received routine midwifery care， while the observation group received responsibility based midwifery care combined with posture management care to compare the effectiveness of midwifery. Results： Compared with the control group， the observation group showed higher satisfaction with nursing care for primiparous women， better delivery outcomes， shorter delivery time， and better pain and psychological well-being after nursing care（P<0.05）. Conclusion： For first-time postpartum women， adopting responsibility based midwifery care combined with posture management has a better effect. The delivery outcome and duration of childbirth have significantly improved， and the psychological status of postpartum women has been improved after nursing. Nursing advantages are prominent， and can be widely promoted in clinical practice.

分娩是指妊娠達到或超过28周（196d）时，胎儿及其附属物品从临产开始至全部从母体娩出的过程[1]。当前，由于人们健康意识的提升，对于分娩安全性的重视度也在不断提升。初产妇由于没有分娩经验，常会出现紧张心理，从而造成分娩过程不顺利，影响分娩结局和分娩安全[2]。故在初产妇围产期为其进行适当的助产护理有着重要的作用。本次择期抽取100例初产妇作为此次调查样本，分析责任制助产护理结合体位管理在分娩中的应用效果。现将结果进行以下说明。

1 资料与方法

1.1 一般资料

观察组产妇50例，年龄18～30岁，平均年龄（25.62±2.01）岁；对照组产妇50例，年龄20～29岁，平均年龄（25.98±2.01）岁。两组一般资料差异较小，具有可比性（P>0.05）。纳入标准：①均为初次妊娠，且在本院进行规律产检的产妇；②胎儿均为单胎且无异常。排除标准：①精神异常；②严重妊娠并发症患者。
