【摘要】 目的：分析巴氯芬在三叉神經痛中的疗效及对降钙素基因相关肽（CGRP）、P物质（SP）水平的影响。方法：收集2019年5月-2020年5月100例于佳木斯大学宏大医院就诊的三叉神经痛患者为研究对象，按照随机数字表法将其分为卡马西平组与巴氯芬组，每组50例。卡马西平组使用卡马西平进行治疗，巴氯芬组使用巴氯芬进行治疗。比较两组五维健康量表（EQ-5D）、视觉模拟评分法（VAS）评分，C反应蛋白（CRP）、白细胞介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）水平，CGRP、SP、神经生长因子（NGF）水平，治疗效果及不良反应发生情况。结果：与卡马西平组相比，巴氯芬组治疗后EQ-5D、VAS评分，CRP、IL-6、TNF-α水平，CGRP、SP、NGF水平均较低（P<0.05）。与卡马西平组相比，巴氯芬组治疗总有效率较高（P<0.05）。与卡马西平组相比，巴氯芬组不良反应发生率较低，但两组比较差异无统计学意义（P>0.05）。结论：巴氯芬在三叉神经痛治疗中可有效缓解患者疼痛感、提高生命质量，降低CGRP、SP水平，缓解机体炎症反应，效果显著。

Efficacy of Baclofen in Trigeminal Neuralgia and Its Influence on CGRP and SP Levels/AI Changsi. //Medical Innovation of China， 2022， 19（16）： 0-081

[Abstract] Objective： To analyze the efficacy of Baclofen in trigeminal neuralgia and its influence on the levels of calcitonin gene-related peptide （CGRP） and substance P （SP）. Method： A total of 100 patients with trigeminal neuralgia treated in Jiamusi University Hongda Hospital from May 2019 to May 2020 were collected as the research objects. They were randomly divided into Carbamazepine group and Baclofen group， 50 cases in each group. Carbamazepine group was treated with Carbamazepine and Baclofen group was treated with Baclofen. Five dimensional health scale （EQ-5D）， visual analog scale （VAS） scores， C reactive protein （CRP）， interleukin-6 （IL-6） and tumor necrosis factor were observed-α （TNF-α）， the levels of CGRP， SP and NGF， the therapeutic effect and the occurrence of adverse reactions were compared between two groups. Result： Compared with Carbamazepine group， the scores of EQ-5D and VAS， the levels of CRP， IL-6 and TNF-α， the levels of CGRP， SP and NGF in Baclofen group were lower （P<0.05）. The total effective rate of Baclofen group was higher than that of Carbamazepine group （P<0.05）. Compared with Carbamazepine group， the incidence of adverse reactions in Baclofen group was lower， but there was no significant difference between two groups （P>0.05）. Conclusion： Baclofen in the treatment of trigeminal neuralgia can effectively alleviate the pain of patients， improve the quality of life， reduce the levels of CGRP and SP， and alleviate the inflammatory reaction of body， the effect is remarkable.

三叉神经痛在临床属于常见的脑神经疾病之一，根据类型不同可分为原发性、继发性三叉神经痛，其中以原发性三叉神经痛最为常见[1]。原发性三叉神经痛临床症状显著，但在临床检查过程中未发现与发病有关的器质性病变;此外，此病主要表现为阵发性疼痛，发作时程度剧烈，对患者的日常生活及工作造成极大困扰[2-3]。药物治疗为三叉神经痛的常见治疗方案，其中卡马西平为常用药物，但其剥脱性皮炎等不良反应严重影响临床用药安全[4]。……
