【摘要】文章总结陈四文主任医师治疗小儿鼻渊的临证经验。陈四文主任认为鼻渊病机本质与苇茎汤病机存在相似之处，皆为痰（湿）热瘀结，并强调本病早期化瘀排脓的重要性。临床上将此方化裁用于治疗小儿鼻渊，无论是急性、慢性，均取得明显疗效，并举隅医案二则，以供临床参考。

Experience of Weijing Decoction in Treating Children Sinusitis

Abstract：This paper summarizes the clinical experience of chief physician Chen Siwen in treating Children Sinusitis. Chen thinks that the essence of the sinusitis is similar to that of weijing decoction， both of which are phlegm （dampness） heat and blood stasis， and emphasizes the importance of removing blood stasis and purulent in the early stage of the disease. Clinically， this prescription has been used to treat Children Sinusitis， both acute and chronic， and has achieved good curative effect. Two medical cases are cited for clinical reference.

鼻渊是以鼻流浊涕为主要临床表现的疾病，相当于现代医学的急、慢性鼻-鼻窦炎。目前西医常用抗生素、鼻喷激素等药物治疗本病，虽短期内可快速缓解患儿症状，但易复发，且可能出现胃肠道不适、过敏、鼻干燥等不良反应^[1]，中医药治疗本病具有其自身优势。陈四文主任医师善用中医药治疗小儿疾病，临床常用苇茎汤加味治疗小儿鼻渊。苇茎汤出自医圣张仲景所著的《金匮要略》中，具有清肺化痰、活血排脓功效，原方主要用于治疗肺痈，陈四文主任根据其病机及临床经验总结出本方应用要点：湿、痰、热、瘀四大病理因素，并在临床中用苇茎汤加味治疗小儿鼻渊，取得明显疗效，现总结如下。

1 苇茎汤病机证治特点及治疗演变

《金匮要略》原方记载：“《千金》苇茎汤，治咳有微热，烦满，胸中甲错，是为肺痈。”指出本方是治疗肺痈的专方。“风舍于肺，其人则咳，口干喘满，咽燥不渴，多浊唾沫，时时振寒。热之所过，血为之凝滞，蓄结痈脓，吐如米粥。”说明肺痈初为外邪客肺，肺失宣降而咳，肺主水，调节全身水液代谢，故当邪犯于肺，肺失宣降，生湿生痰，咳吐浊唾沫。……
