摘要：目的 探讨利用下睑眶隔内脂肪团游离移植泪槽沟达到眶周年轻化的临床观察效果。 方法 广西南宁东方医疗美容医院整形外科采用眶隔脂肪团游离移植泪槽沟治疗眼袋及泪槽凹陷患者69例。术后追踪随访，定期观察临床效果、患者满意度、不良反应及并发症。 结果 对比术前眶周老化状况改善明显，满意率为90%。其中术后出现局部结膜水肿10例、表皮瘀青2例，于术后2周内逐渐缓解;6例因脂肪容量不足仍存在轻度泪槽凹陷;1例因下睑睑板松弛出现不可逆的睑球分离，行下睑睑板悬吊术后治愈;其余患者无不良反应。 结论 眼袋整复术中联合眶隔游离脂肪团移植泪槽的方法可有效预防及改善泪槽凹陷畸形，其手术过程简捷且改善效果明显，术后眼袋复发膨出几率低，患者对术后效果满意。

【 Abstract 】 Objective To investigate the clinical effect of free transplantation of cellulite in the lower eyelid orbital septum into the lacrimal groove to achieve periorbital youth. Methods The plastic surgery department of Nanning Oriental Medical Cosmetology Hospital in Guangxi used orbital cellulite free transplantation to treat 69 patients with eye bags and lacrimal groove depression. Postoperative follow-up and regular observation of clinical effects， patient satisfaction， adverse reactions and complications. Results Compared with the preoperative periorbital aging condition， the improvement was obvious， and the satisfaction rate was 90%. Among them， there were 10 cases of local conjunctival edema and 2 cases of epidermal bruising after the operation， which gradually resolved within 2 weeks after the operation; 6 cases still had mild lacrimal groove depression due to insufficient fat volume; 1 case was irreversible due to laxity of the lower tarsal The eyelid ball was separated and cured after the lower eyelid sling; the rest of the patients had no adverse reactions. Conclusion The method of lacrimal groove transplantation combined with orbital septum free cellulite can effectively prevent and improve the lacrimal groove depression deformity， the surgical process is simple and the improvement effect is obvious， the postoperative pouch recurrence rate is low， the patients are satisfied with the postoperative effect.

泪槽沟凹陷，使人视觉上产生衰老，甚至存有眼袋的现象[1]。而带蒂脂肪移植泪槽沟的眶隔释放祛眼袋术为主流解决和预防眼袋术后泪槽沟凹陷畸形的方法[2]。但是经过观察发现其存在着脂肪移植量不易掌控以及术后脂肪瓣线结松脱、回缩等导致术后眼袋复发的问题[3]。故自2019年3月至2020年12月，廣西南宁东方医疗美容医院整形外科采用眶隔脂肪团游离移植泪槽沟方式治疗眼袋及泪槽沟凹陷患者69例，取得了较满意的效果，现报道如下：
