Study on the prevalence of lower extremity vascular disease and fibrinogen predicting its cut-off value in patients with type 2 diabetes mellitus

[Abstract] Objective The prevalence of lower extremity vascular disease(LEDV) in inpatients with type 2 diabetes mellitus (T2DM) was analyzed, and the correlation between plasma fibrinogen (FIB) and peripheral artery diseases (PAD) in T2DM patients was analyzed to determine whether it can be used as a clinical index to predict LEDV in T2DM patients. Methods A total of 568 inpatients with T2MD were randomly selected from 2015 to 2018 in the Department of Endocrinology and Metabolism of Nanfang Hospital for plasma fibrinogen and ankle-brachial index (ABI). According to ABI results, they were divided into the PAD group (ABI≤0.9) and the non-PAD group (ABI>0.9), each with 56 cases. The plasma fibrinogen characteristics of the two groups were analyzed. Results The prevalence of LEVD in 568 diabetic patients was 9.9%, and there was no correlation with gender,age and fibrinogen. ABI was significantly correlated with fibrinogen(P=0.00).According to the receiver operating curve (ROC), the sensitivity, the specificity, and the optimal cut-off value were 71.4%, 80.4% and 2.69 g/L, respectively. Conclusion Plasma fibrinogen has good sensitivity and specificity for LEVD in diabetic patients.When plasma fibrinogen is more than 2.69 g/L, it indicates that the risk of LEVD is significantly increased.

下肢血管病變是2型糖尿病患者常见大血管并发症,也是2型糖尿病患者截肢及死亡的主要原因之一。早在1992年英国的一项大样本临床研究中就发现,2型糖尿病患者PAD的发病率为23.5%[1],合并糖尿病的PAD患者,更容易出现肢体的溃疡和坏疽,截肢的风险增加[2]。因此,如能寻找预测血管病变的指标,早期及时干预,可有效改善糖尿病足患者生活质量。踝臂指数(Ankle brachial index,ABI)是临床早期诊断下肢血管病变简单无创的一种手段。研究证明,2型糖尿病下肢血管病变与凝血活性异常有密切的关系[3]。纤维蛋白原是凝血活性的重要指标,被认为在2型糖尿病患者的动脉粥样硬化的发生发展中起重要作用。很多研究显示,高纤维蛋白原是PAD的重要危险因素。因此,本研究探讨ABI与纤维蛋白原之间关系,并寻找预测PAD的切点值,以进一步指导临床诊治,为早期预防糖尿病下肢血管病变提供依据,现报道如下。
1 资料与方法
1.1 一般资料
按照1999年美国WHO对糖尿病诊断和分型标准,随机选择2015年5月至2018年3月在南方医院内分泌科住院的2型糖尿病患者568例,男295例(51.9%),平均年龄(54.35±12.68)岁,女273例(48.1%),平均年龄(61.49±12.97)岁。均行ABI检查。诊断标准:由于早期PAD常缺乏典型临床症状,故在本研究中选择ABI为评价指标,以ABI≤0.9作为PAD诊断标准[2]。
1.2 方法
1.2.1 资料收集 记录研究对象姓名、性别、年龄。所有患者均簽署知情同意书。
