【摘 要】 目的：观察中药熏洗联合吲哚美辛三七冰片栓治疗肛门坠胀的临床疗效。方法：选取湿热下注型肛门坠胀患者80例，按随机数字法将患者随机分为对照组（n=40）和观察组（n=40），对照组单纯用吲哚美辛三七冰片栓治疗。观察组采用中药熏洗联合吲哚美辛三七冰片栓治疗，两组治疗共3个疗程（7 d为1个疗程）后进行疗效评价，观察中医证候评分及随访1年复发率情况。结果：观察组总有效率达92.5%。对照组总有效率为80%，两组疗效比较差异具有统计学意义（P<0.05）。两组治疗前中医证候积分差异无统计学意义（P>0.05），治疗后差异具有统计学意义（P<0.05）。两组随访1年内复发率比较差异具有统计学意义（P<0.05）。结论：中药熏洗联合吲哚美辛三七冰片栓对治疗湿热下注型肛门坠胀有显著的作用，值得在临床应用。

Abstract：Objective To explore the clinical observation of Chinese medicine fumigation and washing combined with indomethacin notoginseng borneol suppository in the treatment of anal bloating.Methods Eighty patients with hygrothermal anal bloating who were admitted to Anorectal Hospital of the First Affiliated Hospital of Guizhou University of Traditional Chinese Medicine were randomly divided into observation group （n=40） and control group （n=40） according to the random number method.The observation group was treated with traditional Chinese medicine fumigation combined with indomethacin notoginseng borneol suppository， while the control group was treated with indomethacin notoginseng borneol suppository alone.The curative effect， TCM syndrome score and recurrence rate at 1 year follow-up were evaluated after 3 courses of treatment （7 days as one course） in both groups.Results In the observation group， the total effective rate was 92.5%.In the control group， the total effective rate was 80%.There was a statistically significant difference in the efficacy between the two groups （P<0.05）.There was no significant difference in TCM syndrome scores between the two groups before treatment （P>0.05）， and after treatment （P<0.05）.The recurrence rate within 1 year of follow-up in the two groups was statistically significant（P<0.05）.Conclusion Combination of traditional Chinese medicine fumigation and indomethacin notoginseng borneol suppository has a significant positive effect on the treatment of damp-heat down-injection anal bloating， which is worthy of popularization in clinical practice.

肛門坠胀在肛肠科是常见的、多发的、而且难治性的临床症状，病因及发病机理较为复杂，可以单独存在，也可伴随其他疾病共同存在，一般好发于40～60岁的女性，且男、女均可发病，发病与年龄、职业、饮食、习惯等密切相关[1]。临床常见的表现症状为肛门部下坠及胀满感，或伴有肛门部疼痛，或肛内发热、外阴潮湿感、瘙痒，或伴随里急后重感。也可有肛门异物感、肛门梗阻感，大便不尽感等症状。严重者可放射至腰骶部、臀部及大腿附近。目前由于该疾病病因复杂，病情反复，临床治疗尚无统一标准。笔者采用中药熏洗联合吲哚美辛三七冰片栓治疗湿热下注型肛门坠胀取得显著效果，现报道如下。

1 资料与方法

1.1 一般资料 本研究所有病例均来自贵州中医药大学第一附属医院肛肠病医院2018年1月至2018年10月以肛门坠胀为主诉，且通过四诊合参、综合辨证为“湿热下注”证型的80例门诊患者。……
