数字减影血管造影联合神经介入溶栓术治疗缺血性脑血管疾病的效果及对脑血流动力学和血管内皮功能的影响

[摘要] 目的 探討数字减影血管造影联合神经介入溶栓术治疗缺血性脑血管疾病的效果及对脑血流动力学和血管内皮功能的影响。 方法 选取2018年9月至2019年9月缺血性脑血管患者160例,随机分为试验组与对照组,每组各80例。对照组患者仅进行常规静脉溶栓治疗,试验组则通过数字减影血管造影联合神经介入溶栓术治疗,比较两组患者近期治疗疗效、脑血流动力学和血管内皮功能指标变化情况。 结果 两组患者的近期治疗疗效、脑血流动力学和血管内皮功能指标变化比较,差异有统计学意义(P<0.05)。其中试验组患者痊愈、显效和有效比例高于对照组,治疗后,试验组患者脑血流动力学指标Qm值大于对照组,而WV、ZCV和RV值小于对照组,血管内皮功能指标ABI和PWV值小于对照组,PWV值则较大。 结论 数字减影血管造影联合神经介入溶栓术治疗缺血性脑血管疾病的效果较仅使用常规静脉溶栓治疗效果好,不仅能够改善脑血流动力学,同时还能够减轻血管内皮功能的损伤,值得临床广泛推广使用。

Efficacy of digital subtraction angiography combined with neural interventional thrombolysis on ischemic cerebrovascular disease and its impacts on cerebral hemodynamics and vascular endothelial function

[Abstract] Objective To explore the efficacy of digital subtraction angiography(DSA) combined with neural interventional thrombolysis on ischemic cerebrovascular diseases and its impacts on cerebral hemodynamics and vascular endothelial function. Methods A total of 160 patients with ischemic cerebrovascular diseases from September 2018 to September 2019 were equally divided into the experimental group (n=80) and the control group(n=80). The patients in the control group were only treated with conventional intravenous thrombolysis, while the patients in the experimental group were treated with DSA combined with neural interventional thrombolysis. Short-term therapeutic efficacies, cerebral hemodynamics and changes of vascular endothelial function indexes of the patients were compared and analyzed. Results There were statistically significant differences in short-term treatment efficacy, cerebral hemodynamics and changes in vascular endothelial function indexes between the experimental group and the control group(P<0.05). Among them, the rates of recovery, marked efficacy and efficacy rate of patients in the experimental group were higher than those in the control group. After treatment, the Qm value of cerebral hemodynamics index of patients in the experimental group was higher than that in the control group, while the values of WV, ZCV and RV were lower than those in the control group. The values of ABI and PWV of vascular endothelial function indexes were lower than those in the control group, while the PWV value was higher. Conclusion DSA combined with neural interventional thrombolysis in the treatment of ischemic cerebrovascular disease has a better efficacy than the conventional intravenous thrombolysis therapy. It can not only improve cerebral hemodynamics, but also reduce the damage of vascular endothelial function, which is worthy of extensive clinical promotion and application.

伴随人们生活水平的不断提升,人们的生活和工作压力也在不断提升,进而增加了各种疾病的发生率,尤其是缺血性脑血管疾病,对患者功能障碍造成严重威胁,导致各种并发症的发生,不但增加社会和家庭负担,也使患者承受较大痛苦[1-2]。作为脑部疾病的缺血性脑血管疾病在临床上极为常见该病的治疗在临床上越来越受到关注。数字减影血管造影(Digital subtraction angiography,DSA)即可对缺血性脑血管疾病进行治疗,且应用越来越多[3-4],而目前缺血性脑血管疾病患者的治疗多采用神经介入溶栓术治疗,DSA联合神经介入溶栓术治疗则相对较少。因此,本试验通过通过数字减影血管造影联合神经介入溶栓术对缺血性脑血管患者进行治疗,研究对脑血流动力学和血管内皮功能的影响,以期在减轻缺血性脑血管患者痛苦的基础上,为进一步治疗提供科学的理论依据和奠定良好的理论依据,现报道如下。……
