[摘要]目的：探究無托槽隐形矫治对错牙合畸形患者牙周健康的影响。方法：选取笔者医院2017年5月-2019年1月收治的58例错牙合畸形患者为研究对象，依据随机数字表法分组，每组29例。对照组采取传统固定矫治，观察组采取无托槽隐形矫治。比较两组治疗效果、治疗前后牙周指数[菌斑指数（PLI）、牙龈指数（GI）、龈沟出血指数（SBI）]、牙周炎性损伤指标[天冬氨酸转氨酶（AST）、碱性磷酸酶（ALP）]及并发症发生情况。结果：观察组治疗总有效率93.10%，与对照组89.66%相比，差异无统计学意义（P>0.05）;治疗6个月后，观察组PLI、GI、SBI低于对照组（P<0.05），观察组AST、ALP低于对照组（P<0.05）;观察组并发症发生率10.34%，低于对照组的34.48%（P<0.05）。结论：无托槽隐形矫治错牙合畸形患者疗效显著，可改善牙周健康情况，减轻牙周炎性损伤，且并发症发生率较低。

Effect of Invisible Bracketless Correction on Periodontal Health of Patients with Malocclusion Deformity

Abstract：Objective  To investigate the effect of invisible bracketless correction on periodontal health of patients with malocclusion deformity. Methods  A total of 58 patients with malocclusion deformity who were treated in our hospital from May 2017 to January 2019 were selected as the research objects， they were grouped according to the random number table method， with 29 cases in each group. The control group was treated with traditional fixation， and the observation group was treated with bracketless invisibility. The treatment effects， periodontal parameters [plaque index （PLI）， gingival index （GI）， sulcus bleeding index （SBI）]， periodontal inflammatory damage indicators [aspartate aminotransferase （AST）， alkaline phosphatase （ALP）] before and after treatment， and complications were compared between the two groups. Results  The total effective rate of treatment in the observation group was 93.10% and 89.66%  in the control group， the difference was not statistically significant （P>0.05）. At 6 months of treatment， the PLI， GI，and SBI of the observation group were lower than those of the control group （P<0.05），the AST and ALP of the observation group were lower than the control group at 6 months of treatment （P<0.05）.The incidence of complications in the observation group was 10.34% lower than that in the control group 34.48% （P<0.05）. Conclusion  Invisible braces without brackets are effective in treating patients with malocclusion deformity， which can improve periodontal health， reduce periodontal inflammatory damage， and have a lower incidence of complications.

错牙合畸形可导致牙齿排列不齐、上下牙弓牙合关系异常，不仅影响咀嚼、語言功能，亦严重影响牙齿及口腔美观度[1]。正畸治疗是临床纠正错牙合畸形的主要手段，传统的固定矫治器具有高效、精准等优点，但其结构复杂、需长期佩戴，可能增加牙周病发生风险，且美观度较差[2]。无托槽隐形矫治技术属于集口腔医学、生物医学、计算机科学及快速成型技术等多学科于一身的交叉学科，具有安装便捷、自行摘戴及舒适等特点[3]。目前，正畸医生普遍认为，无托槽隐形矫治更有利于维护牙周健康[4]。但有研究报道，隐形矫治器每天佩戴时间较长，影响口腔自洁能力，加之其一体式覆盖可能压迫牙龈，从而导致牙周受损[5]。因此，关于无托槽隐形矫治对牙周健康的影响仍有待进一步验证。基于……
