CAD/CAM玻璃瓷嵌体对磨牙龋损的修复效果及预后随访研究

[摘要]目的:探讨计算机辅助设计与制造(Computer aided design and manufacture,CAD/CAM)玻璃瓷嵌体对磨牙龋损的修复效果及预后影响。方法:选取2015年2月-2018年2月笔者医院收治的120例磨牙龋损患者,将所有患者随机分为两组。对照组:60例,给予纳米树脂填充修复;观察组:60例,给予CAD/CAM玻璃瓷嵌体修复。比较两组患者修复后成功率及预后情况。结果:观察组患者修复后成功率95.00%,明显高于对照组的80.00%,观察组修复后缺陷率3.33%,明显低于对照组的15.00%,差异有统计学意义(P<0.05)。观察组修复后牙髓炎、继发龋及修复体脱落发生率均低于对照组(1.67%vs 11.67%、1.67%vs13.33%、1.67%vs11.67%),差异有统计学意义(P<0.05)。结论:CAD/CAM玻璃瓷嵌体对磨牙龋损的修复效果显著,可降低修复体脱落率,改善并发症发生情况及预后。

A Follow-up Study on the Effect and Prognosis of Dental Caries Repaired by CAD/CAM Glass-ceramic Inlay

Abstract: Objective  To investigate the effect of computer aided design and manufacture (CAD/CAM) glass-ceramic inlay on the restoration and prognosis of molar caries. Methods 120 patients with molar caries admitted to our hospital from February 2015 to February 2018 were selected. All patients were randomly divided into two groups, 60 cases in the control group were repaired with nano-resin filling, and 60 cases in the observation group were repaired with CAD/CAM glass-ceramic inlay. The success rate and prognosis of the two groups were observed and compared. Results  The success rate of the observation group were 95.00% higher than that of the control group 80.00%,the defect rate of the observation group were 3.33% lower than that of the control group 15.00%,the difference were statistically significant(P<0.05).The incidences of pulpitis, secondary caries and prosthetic exfoliation in the observation group were lower than those in the control group (1.67% vs11.67%, 1.67% vs 13.33%, 1.67% vs11.67%), the difference was statistically significant (P<0.05). Conclusion  CAD/CAM glass-ceramic inlay has a remarkable effect on the restoration of molar caries. It can reduce the loss rate of restoration,improve the occurrence of complications, and improve the prognosis.

齲病是对人类口腔健康具有重要危害的常见疾病。随着社会的不断发展,人类生活习惯和饮食结构的不断变化,龋病的发生率呈不断上升趋势[1]。有研究表示[2],目前部分发达国家经过一些综合性防范措施,已逐渐控制住龋病的发病率,但对于一些发展中国家,随着人们食糖消费量的不断上升,饮食结构的不断变化,龋病的发病率依然在不断上升。磨牙是人类口腔中的重要器官之一,不仅在保持面部协调美观、语言及发音等方面具有重要作用,还承担着咀嚼的重要作用[3]。由于口腔内磨牙特有的结构特征,磨牙龋损是临床较为常见的牙齿损伤[4]。既往对于磨牙龋损多使用修复治疗,其修复材料多种多样,常用的有纳米树脂、玻璃离子水门汀等,均有一定效果,但经大量实践表明[5],其预后不佳,修复成功率较低。……
