[摘要]目的：探討黄金微针射频联合重组牛碱性成纤维细胞生长因子（rb-bFGF）凝胶治疗面部痤疮瘢痕的临床疗效。方法：前瞻性选择2017年1月-2019年1月笔者医院诊治的92例面部痤疮瘢痕患者，按照入组奇、偶顺序将患者随机分为两组。对照组：46例，采用黄金微针射频治疗;观察组：46例，采用黄金微针射频联合rb-bFGF凝胶治疗。比较两组疗效、结痂时间、红斑持续时间、痤疮瘢痕评估量表（ECCA）权重评分、瘢痕基底深度、皮肤屏障功能和不良反应差异。结果：观察组治疗总有效率高于对照组（P<0.05），结痂时间、红斑持续时间均短于对照组（P<0.05）。两组治疗后ECCA权重评分、瘢痕基底深度、经皮水分丢失量、pH值低于治疗前（P<0.05），皮肤含水量高于治疗前（P<0.05）。观察组治疗后ECCA权重评分、瘢痕基底深度、经皮水分丢失量、pH值低于对照组（P<0.05），皮肤含水量高于对照组（P<0.05）。观察组治疗期间不良反应发生率低于对照组（P<0.05）。结论：黄金微针射频联合rb-bFGF凝胶治疗可改善面部痤疮瘢痕症状，减轻瘢痕基底深度，增加皮肤屏障功能，提高治疗效果，降低不良反应。

A Clinical Study of Gold Microneedle Radiofrequency Combined Recombinant Bovine-Basic Fibroblast Growth Factor Gel in the Treatment of Facial Acne Scar

Abstract： Objective To investigate the clinical effect of gold microneedle radiofrequency combined with recombinant bovine-basic fibroblast growth factor （rb-bFGF） gel in the treatment of facial acne scar. Methods 92 patients with facial acne scar diagnosed and treated in our hospital from January 2017 to January 2019 were prospectively selected and randomly divided into two groups according to the odd and even order of enrollment. The control group （46 cases） was treated with gold microneedle radiofrequency， and the observation group （46 cases） was treated with gold microneedle radiofrequency combined with rb-bFGF gel. The efficacy， scab time， erythema duration， acne scar assessment scale （ECCA） weight score， scar base depth， skin barrier function and adverse reactions were compared between the two groups. Results The total effective rate of treatment in the observation group was higher than that in the control group （P<0.05）， and the scab time and erythema duration were shorter than that in the control group （P<0.05）. After treatment， ECCA weight score， scar base depth， transdermal water loss and PH value of the two groups were lower than before treatment （P<0.05）， and the skin moisture content was higher than before treatment （P<0.05）. After treatment， the weight score of ECCA， scar base depth， transdermal water loss， pH value of the observation group were lower than those of the control group （P<0.05）， and skin moisture content was higher than that of the control group （P<0.05）. The incidence of adverse reactions in the observation group was lower than that in the control group （P<0.05）. Conclusion  Gold microneedle radiofrequency combined rb-bFGF gel treatment can improve facial acne scar symptoms， reduce scar base depth， increase skin barrier function， improve treatment effect and reduce adverse reactions.

面部痤疮瘢痕是痤疮恢复后或治疗不当引起的凹陷性萎缩性瘢痕，呈片状、斑块状分布，凹凸不平，颜色较深，极度影响美观和患者心理健康[1-3]。临床治疗面部痤疮瘢痕的方法有瘢痕膏、冷冻、磨削、化学剥脱、组织充填、激光及外科治疗等，但是疗效均不明显，安全性差。CO2点阵激光是治疗痤疮瘢痕的主要物理治疗方法，但是治疗后痤疮数量、面积及色素沉着等问题仍较突出[4-5]，为寻求更为理想和有效的治疗方案，笔者医院引进黄金微针射频治疗技术，该技术具有能量稳定、作用深、损伤小及操作简单的优势[6]，可解决多种面部皮肤问题，但单独应用黄金微针效果仍不理想，重组牛碱性成纤维细胞生长因子（Recombinant bovine-basic fibroblast growth factor，rb-bFGF）具有促进基底膜组织再生和新生血管形成作用，利于表皮细胞的修复，本研究将黄金微针射频治疗技术联合rb-bFGF治疗面部痤疮瘢痕，获得了理想效果，现将结果报道如下。……
