[摘要]目的：介绍眼周年轻化的“画框”美学理念，探讨结膜入路下睑袋去除联合自体脂肪填充的下睑年轻化效果。方法：收集2018年1月-2019年10月于笔者科室行结膜入路下睑袋去除联合自体脂肪填充中面部和泪沟的患者，采用眼周“画框”理念对术后效果进行评价。结果：本组共382例患者，随访3～12个月，患者术后睑袋、泪沟改善，眼周“画框”减小，获得下睑年轻化效果。结论：眼周年轻化的“画框”美学理念适用于我国人群，结膜入路下睑袋去除联合自体脂肪填充的手术方法遵循此美学理念，下睑年轻化效果可靠。

Application of Conjunctival Approach to Lower Eyelid Blepharoplasty Combined with Autologous Fat Filling in Lower Eyelid Rejuvenation

Abstract： Objective  To introduce the artistic model of Benslimanes Frame Concept for orbital rejuvenation， and discusses the effect of conjunctival approach to lower eyelid blepharoplasty combined with autologous fat filling on lower eyelid rejuvenation. Methods  From January 2018 to October 2019， patients with conjunctival approach to lower eyelid blepharoplasty combined with autologous fat filling in the middle face and lacrimal groove in our department were collected. The Benslimanes Frame Concept， an orbital artistic model， was used to evaluate rejuvenation effects of the operation. Results  A total of 382 patients received the surgery. The follow-up period ranged from 3 to 12 months. Post-operation， effacement of the eyelid bag and tear trough deformity was achieved， and low-eyelid youthful appearance by narrowing the frame of peri-orbital shadows was restored. Conclusion  The Benslimanes Frame Concept is applicable for Chinese population. Conjunctival approach to lower eyelid blepharoplasty combined with autologous fat filling operation method follows this aesthetic concept， lower eyelid rejuvenation effect is reliable.

年轻人群下睑衰老主要表现为睑袋膨隆、泪沟凹陷。相应的，年轻人下睑年轻化手术则遵循以解决这些问题为目标，手术方式包括经结膜眶隔脂肪去除[1]、经结膜眶隔脂肪释放[2-3]或眶隔脂肪游离回填[4-5]等。近来有学者从美学角度出发，提出眼周年轻化的“画框”理念（Frame Concept），此理念对下睑年轻化的手术目标具有新的指导意义[6-7]。在此理念指导下，笔者自2018年开始行结膜入路下睑袋去除联合中面部及泪沟自体脂肪填充术，术后效果良好，报道如下。

1  资料和方法

1.1 一般资料：2018年1月-2019年10月，于笔者科室行结膜入路下睑袋去除联合中面部和泪沟自体脂肪填充术，并且至少于术后3个月返院复查，手术前后照片资料完整的患者，纳入本研究。本次共382例患者，男性13例，女性369例，年龄18～37岁，平均31岁，均为初次手术患者。……
