摘要医患互动的质量决定患者的就诊满意度、影响医生对病情判断的准确性。医患互动过程分析系统（roter interaction analysis system， RIAS）是针对语言行为的分类系统，用于评价互动过程中的话语结构。RIAS不仅关注生物医学信息还聚焦于社会心理信息的传递。青光眼是一类典型的生理心理性疾病。医生在关注青光眼患者生理改变的同时，更要关注患者的心理状态在发病和诊疗中的作用。本文总结了苏州大学附属第一医院眼科临床教学的带教方案，强调了医患互动过程分析系统在青光眼诊疗中的辅助作用，并期待推广。

Application of Doctor-patient Interaction Process Analysis System in Glaucoma Teaching

AbstractThe quality of doctor-patient interaction determines the patient’s satisfaction and affects the accuracy of doctor’s judgment. RIAS is a classification system for language behavior， which is used to evaluate the discourse structure in the process of interaction. RIAS not only focuses on biomedical information， but also focuses on the transmissionofpsychosocialinformation.Glaucomaisa typicalphysiologicalandpsychologicaldisease.Whilepaying attention to the physiological changes of glaucoma patients， doctors should pay more attention to the role of patients’ psychological state in the pathogenesis and diagnosis and treatment. This paper summarizes the clinical teaching plan ofophthalmology intheFirstAffiliatedHospitalofSoochowUniversity，emphasizesthe auxiliaryroleofdoctor-patient interactive process analysis system in glaucoma diagnosis and treatment， and looks forward to promoting it.

1青光眼的發病机制及诊疗过程中患者的心理、情绪特点

据统计，至2020年全球范围内有多达7600万的青光眼患者，预计至2040年一以数目将增长超过1亿1千万，是发病率排名第二的致盲性眼病，排名第一的不可逆性致盲性眼病。超过80%的青光眼患者分布在亚洲，同时，患者对该疾病的知晓率较低，甚至不足50%。[1]

由于早期青光眼以周边视野缺损为主要表现，不伴有中心视力降低，所以大多数患者难以在早期出现视力降低的明显症状，当累及中心视力而就诊时疾病往往已发展至中晚期；或者患者在青光眼急性大发作出现剧烈眼胀眼痛症状时至医院就诊。由于大多数患者发病时病情较严重，这就导致了其本身多数伴随严重紧张或者焦虑的情绪。在这种条件下，如就诊时医生沟通缺乏技巧，直接告知其患病的终点是不可逆性盲，患者极可能发生情绪失控。……
