[摘要]目的：观察在上颌窦提升植骨术中使用明胶海绵作为植骨填充材料的临床应用效果。方法：共计纳入35例患者为观察对象，将明胶海绵剪碎后混合小牛骨粉作为植骨材料为25例患者上颌窦进行植骨，另10例患者，使用大块明胶海绵填充于上颌窦中无种植体植入的提升空间。术后随访3年，观察患者上颌窦提升的成功率及种植体植入后的成活率。结果：在35例上颌窦外提升手术中植入57颗种植体，上颌窦外提升植骨术成功率100.0%，共有2例种植体发生松动脱落，种植体存留率96.5%。结论：明胶海绵可以作为辅助填充材料应用于上颌窦外提升植骨中。

The Clinical Application of Gelatin Sponge in Maxillary Sinus Elevation and Bone Grafting

Abstract：Objective The purpose of study was to evaluate the clinical results when gelatin sponge was used as an auxiliary grafting material in lateral sinus floor elevation surgery. Methods 35 selected patients with 35 lateral sinus floor elevation surgeries were enrolled in the study. Gelatin sponge was cut into small chunks， and mixed with bovine bone particles， the mixture was then grafted in 25 sinus elevation surgeries; in another 10 patients sinus， big chunks of gelatin sponge was used to graft the sinus space in the posterior area where there was no implant inserted. The survival rate of implants and the success rate of sinus floor elevation was calculated in 3 years follow-up. Results 57 implants were inserted in 35 sinus surgeries in 35 patients. The success rate of sinus floor elevation surgeries was 100.0%， the survival rate of implants was 96.5%. Conclusion The results showed that gelatin sponge was safe to be used as grafting material when mixed with bovine bone particles during maxillary sinus elevation surgery.

在上颌后牙区进行牙种植体植入时，常会面临因骨吸收和上颌窦气化导致的骨高度不足的情况[1]。上颌窦提升植骨术可以有效解决上颌后牙区骨高度不足的问题，是目前较安全有效的临床术式[2-4]。常用的植骨材料主要有：自体骨、同种异体骨、异种骨及生物合成骨等。小牛骨粉虽然在材料性能上仍不完美，例如无骨诱导性能[5-6]，但仍然是目前临床应用广泛且安全的植骨材料之一[7-8]。明胶海绵是常用的拔牙术后止血材料，呈黃色或白色，质地柔软多孔，可被降解吸收。明胶海绵吸血后可以促进血小板聚集破裂，并形成凝血块，从而促进创伤愈合。……
