【摘要】 目的：研究水膠体敷料和护肤粉联合皮肤保护膜两组种护理干预方法应用于肿瘤重症中重度失禁性皮炎患者的临床疗效。方法：选取笔者所在医院2018年10月-2019年10月收治的肿瘤重症合并中重度失禁性皮炎患者65例，按照随机数字表法分为观察组（33例）和对照组（32例）。在常规皮肤护理的基础上，观察组给予水胶体敷料皮肤护理措施，视情况1～3 d更换1次;对照组给予护肤粉联合皮肤保护膜皮肤护理措施，3次/d;两组护理连续10 d。对两组皮炎改善情况、疼痛缓解时间、皮损愈合时间及护理满意度进行比较分析。结果：观察组疼痛缓解时间、皮损愈合时间均短于对照组，差异均有统计学意义（P<0.05）;观察组护理满意度评分高于对照组，差异有统计学意义（P<0.05）;两组患者皮炎改善情况比较差异无统计学意义（P>0.05）。结论：水胶体敷料应用于肿瘤重症中重度失禁性皮炎患者能较快缓解患者疼痛，缩短皮损愈合时间，提高患者护理满意度，具有良好的临床价值。

[Abstract] Objective： To study the clinical effect of two kinds of nursing intervention methods of hydrocolloid dressing and skin care powder combined with skin protective film on patients with severe incontinence dermatitis. Method： From October 2018 to October 2019， 65 patients with severe tumor combined with moderate and severe incontinence dermatitis were selected and divided into the observation group （33 cases） and the control group （32 cases） according to the random number table. On the basis of conventional skin care， the observation group was given skin care measures of hydrocolloid dressing， which was changed once every 1-3 days as the case may be， the control group was given skin care measures of skin powder combined with skin protective film， which was three times a day， the two groups were given 10 days of continuous care. The improvement of dermatitis， pain relief time， skin healing time and nursing satisfaction were compared between the two groups. Result： The pain relief time and skin healing time in the observation group were shorter than those in the control group， and the differences were statistically significant （P<0.05）. The nursing satisfaction score in the observation group was higher than that in the control group， and the difference was statistically significant （P<0.05）. There was no significant difference in dermatitis improvement between the two groups （P>0.05）. Conclusion： The application of hydrocolloid dressing in the patients with severe incontinence dermatitis can quickly relieve the pain， shorten the healing time of skin lesions， and improve the patients' nursing satisfaction， which has good clinical value.

失禁性皮炎（incontinence-assoxiated dermatitis，IAD）是指皮肤长期或反复暴露于尿液和粪便中所造成的炎症，可伴有水疱或者皮肤破损，使患者产生不同程度的疼痛感，是肿瘤重症失禁患者常见的皮肤并发症[1]。中重度失禁性皮炎表现为病变部位皮层缺损，可伴有渗出或出血，严重影响患者的身心健康。目前临床主要采用护肤粉联合皮肤保护膜对患者进行干预治疗，但其治疗缓慢，护理频繁，影响患者的舒适度，水胶体敷料是在湿性愈合理论指导下研发的新型敷料，被广泛应用于慢性伤口如压疮和静脉溃疡的治疗[2]。……
