[摘要]目的 探讨快速康复外科（FTS）理念在中老年髋周骨折患者护理的应用效果。方法 选取2017年1月～2018年12月我院收治的68例中老年髋周骨折患者，按照护理方式的不同分为对照组与观察组，每组各34例。对照组采用常规护理，出院后常规随访;观察组采用FTS，出院后進行延伸护理。比较两组的功能康复情况、术后并发症发生情况、住院时间、生活质量。结果 观察组出院时和出院后1个月康复评分高于对照组，住院时间短于对照组，差异均有统计学意义（P<0.05）;观察组出院时和出院后1个月Barthel指数高于对照组，差异均有统计学意义（P<0.05）;观察组并发症总发生率低于对照组，差异有统计学意义（P<0.05）。结论 在中老年髋周骨折患者护理中，在FTS理念护理，能有效增进患者的功能康复，缩减患者的住院时间，减少并发症的发生率，提高患者的生活能力，值得临床推广应用。

Application of the concept of rapid rehabilitation surgery in the nursing of middle-aged and elderly patients with peri-hip fracture

[Abstract] Objective To explore the application effect of the concept of rapid rehabilitation surgery （FTS） in the nursing of middle-aged and elderly patients with peri-hip fracture. Methods A total of 68 middle-aged and elderly patients with peri-hip fracture in our hospital from January 2017 to December 2018 were selected. They were divided into control group and observation group by nursing mode， 34 cases in each group. The control group was adopted routine nursing and routine follow-up after discharge. The observation group was adopted FTS nursing and extended nursing after discharge. Functional rehabilitation， postoperative complications， hospitalization days and quality of life were compared between the two groups. Results The rehabilitation score of the observation group at discharge and 1 month after discharge were higher than those of the control group， and the length of hospitalization was shorter than that in the control group， with statistical differences （P<0.05）; the Barthel index of the observation group at discharge and 1 month after discharge were higher than those of the control group， with statistical differences （P<0.05）; the total incidence of complications in the observation group was lower than that in the control group， and the difference was statistically significant （P<0.05）. Conclusion In the nursing of middle-aged and elderly patients with peri-hip fracture， nursing under the guidance of the concept of FTS nursing can effectively improve the functional rehabilitation of patients， shorten the length of hospital stay， reduce the incidence of complications and improve the living ability， worthy of clinical application.

FTS被称为快速康复外科，最初由丹麥教授Henrik Kehlet提出，以循证医学为依据，在整个围术期，实施一系列优化治疗与护理措施，减缓手术及心理带来创伤指数，减短住院日数、减少住院费用、降低并发症发生，保障患者生命安全，促进患者术后快速康复[1-2]。FTS理念指术前、术中及术后，采用经证实有效方法，减少手术应激反应，控制术后并发症，促使患者快速康复[3]。FTS理念是一种新型护理模式，近年来，国内已将FTS理念用于临床各领域。我院对中老年髋周骨折患者使用FTS理念护理，术前超前教育，术后对患者制定个体化康复训练计划，出院后电话随访、家庭访视和微信相结合方式进行延伸护理。从入院直至出院后实施全方位、专业整体康复措施，取得良好效果，现报道如下。
