不同剂量运动对高尿酸血症影响的试验研究

摘  要:研究目的  分析不同剂量运动对高尿酸血症(HUA)患者血尿酸水平及相关指标的影响,进而为HUA患者进行运动锻炼提供安全有效的运动方案。研究方法  以网上报名的方式在南京地区筛选未采用药物治疗的HUA患者18名,进行为期三个月不同剂量的慢跑干预试验,受试者被随机分为高剂量组和低剂量组各9人,在规定单次运动时间和运動强度双因素后,高剂量组进行每周5次慢跑训练,低剂量组进行每周3次慢跑训练,采集受试者试验前后的血检指标、身体成分和最大摄氧量进行对比分析。研究结果  不同跑量间训练周期前后,高剂量组FPG(P=0.041)、体重(P=0.033)、脂肪量(P=0.001)、BMI(P=0.032),均P<0.05,差异有统计学意义。高剂量组和低剂量组其他数据均无统计学差异,但从数值上看,UA、Cr、TG、ALP、AST、ALT和VO2max均有改善趋势,并且高剂量组趋势更加明显。研究结论  在有氧强度下,运动剂量与治疗效果成正相关,一定程度上降低了HUA患者的UA水平,改善了肾脏功能及相关生化指标,进而使身体机能产生整体性变化。

Abstract: Objective: To analyze the effects of different doses of exercise on serum uric acid level and related indicators in patients with hyperuricemia (HUA), and to provide a safe and effective exercise program for patients with HUA. Methods: Eighteen patients with HUA who did not take medication were screened by online registration in Nanjing area for a three-month jogging intervention experiment. The subjects were randomly divided into high-dose group and low-dose group with 9 in each group. After prescribing single exercise time and exercise intensity, the high-dose group carried out each jogging intervention for three months. Jogging training 5 times a week, jogging training 3 times a week in low dose group. Blood test indexes, body composition and maximum oxygen uptake before and after the experiment were collected and compared. The results showed that before and after the training cycle, the FPG(P=0.041),body weight(P=0.033),fat(P=0.001),BMI(P=0.032) in the high dose group were all P<0.05, the difference was statistically significant. There was no significant difference in other data between the high-dose group and the low-dose group, but from the numerical point of view, UA, Cr, TG, ALP, AST, ALT and VO2max were all improved, and the trend of the high-dose group was more obvious.Research conclusion: Under aerobic intensity, exercise dose is positively correlated with therapeutic effect, which reduces UA level in HUA patients to a certain extent, improves kidney function and related biochemical indicators, and then makes overall changes in body function.

HUA是指人体由于嘌呤代谢紊乱造成的血中尿酸生成增多和(或)排泄减少的一种病症[1],若长期得不到改善,会致使尿酸盐生成过多在关节内沉积,极易引起痛风及其一系列的并发症[2]。随着相关领域学者对HUA和痛风研究的不断深入,对于症状的划分和治疗的方法也愈加清晰、多样、全面。

近年来,越来越多的国内外学者针对运动与HUA或痛风之间的量效关系,进行了研究、归纳总结,但是运动对痛风或高尿酸血症是否具有康复、治疗作用,学者的观点各不相同,存在较大争议性[3]。
