国产利奈唑胺治疗革兰阳性球菌的效果及成本分析

【摘 要】目的:探讨国产利奈唑胺治疗革兰阳性球菌感染的效果及成本。方法:纳入2017年1月-2018年1月于本院接受国产或进口利奈唑胺治疗的74例革兰阳性球菌感染患者,根据抗菌药物不同,分为观察组53例(国产利奈唑胺)与对照组21例(进口利奈唑胺)。分析两组患者临床有效率、10d细菌清除率、药物不良反应、30d病死率及药品成本等。结果:观察组与对照组有效率[81%(43/53)vs 86%(18/21)]、10d细菌清除率[83%(44/53)vs 76%(16/21)]、不良反应[22.6%(12/53)vs 23.8%(5/21)]、30d死亡率[3.8%(2/53)vs 4.8%(1/21)],差异均无统计学意义(P>0.05);观察组药品费用低于对照组(7985.61元< 9951.54元,P =0.000)。结论:国产利奈唑胺治疗革兰阳性球菌感染效果確切、安全性良好、成本低。

Domestic linidazolamide in the treatment of Gram-positive coccus infections:the analysis
of clinical and cost-effectiveness

【Abstract】Objective: To investigate the clinical and cost-effectiveness of domestic linezolid in the treatment of infections induced by gram-positive coccus . Methods: Totally of 74 cases of gram-positive cocci bacteremia patients, from January 2017 to January 2018 in the hospital, who were treated with domestic linezolid or imported linezolid,were included. According to the different antimicrobial agents, 53 patients were divided into two groups.: the observation group treated with domestic linezolid(53cases) and the control group treated with imported linezolid(21cases). The following data were collected and compared: clinical efficiency, bacterial clearance rate(10d), adverse events, fatality rate (30d) and cost-effectiveness. Results: the clinical effective were 81% (43/53) in the observation group and86%(18/21) in the control group; bacterial clearance rate in10 days were 83% (44/53) and 76%(16/21), adverse events were 22.6% (12/53) and 23.8%(5/21)], and mortality during 30d were 3.8% (2/53 )and 4.8%(1/21)] in the two groups ,respectively(P>0.05). Drug costs in the observation group were lower than those in the control group(7985.61< 9951.54, P =0.000). Conclusion:domestic linidazolamide is an option with high clinical efficacy, good safety and low er cost in the treatment of gram-positive coccus infections.

革兰阳性球菌如金黄色葡萄球菌(Staphylococcus aureus,SA)、耐甲氧西林的金黄色葡萄球菌(methicillin-resistant staphylococcus aureus,MRSA)、肠球菌等,耐药性强且感染率逐年上升,容易引起复杂的皮肤、软组织感染、脑膜炎、心内膜炎等,其中MRSA,是引起血液感染的主要原因,并致高发病率和死亡率[1]。在美国每年约有25万例患者因血流感染死亡,其中20%由SA引起[2]。住院患者中超过50%的SA感染由MRSA引起[3]。MRSA感染患者在30天内的死亡率在30-40%[4]。肠球菌的院内感染正在增加,根据美国疾病控制和预防中心的国家医疗安全网络(the CDCs National Healthcare Safety Network ,NHSN)数据显示,在美国肠球菌引起13.9%院内感染,是第二大常见的病原体[5]。临床肠球菌感染中,在欧洲和美国越来越多的医院,耐万古霉素肠球菌(Vancomancoycin-Resistant Enterococcus,VRE)逐年上升[6]。Vega和Dowzicky研究显示[7],分离出的屎肠球菌40.8% (235/576) 和粪肠球菌1.6%(33/2004)对万古霉素耐药。利奈唑胺作为万古霉素的替代治疗出现,其有效性及安全性已得到国内外研究充分证明[8]。由于……
