摘 要：建立社区健康档案是基础医疗服务发展的必然趋势，健康档案是居民健康情况的有效反映，为居民卫生健康服务提供依据。基于此，本文以大数据时代下社区居民健康档案作为研究对象，分析社区居民健康档案的基本特征，发现档案管理存在的问题，分别从社区健康档案宣传、医疗管理机制、信息化建设以及人才培训等方面阐述了社区居民健康档案管理对策。

Abstract：Establishing community health archives is the inevitable trend of the development of basic medical services. Health archives are an effective reflection of residents’health conditions and provide basis for residents’health services. Based on this，this paper takes community residents’health archives in the era of big data as the research object，analyses the basic characteristics of community residents’health archives，finds out the problems of archives management，and elaborates the countermeasures of community residents’health archives management from the aspects of community health archives publicity，medical management mechanism，information construction and personnel training.

0 引 言

社區居民健康档案包含居民的基本信息资料、健康问题、周期内健康检查记录以及住院或转诊记录，有的还包括家庭健康档案，通过对社区内居民健康情况的分析与了解，可以筛查疾病高危人群，对其进行有针对性的健康指导与用药指导，有效预防慢性病。社区居民健康档案是医生与护士们展开诊疗的重要参考，通过健康档案查询患者病历资料，方便了解和掌握患者的健康问题。

1 社区居民健康档案的基本特征

社区居民健康档案的基本特征具体如下：（1）以人为本。社区居民健康档案的建立与管理以居民健康为核心，满足社区居民对保健健康的需求；（2）内容完整、重点突出。建立社区居民健康档案，所有的信息记录会贯穿一个人的生命全程，不仅有疾病诊断与治疗全过程，还有心理健康与机体健康情况，健康不仅代表一个人没有疾病，也代表自身与心理保持良好状态。……
