PDK-1蛋白在胃癌组织中的表达及临床意义

【摘要】 目的:研究并分析PDK-1蛋白在胃癌組织中的表达及临床意义,为胃癌的治疗提供新的思路。方法:选取2010年1月-2012年1月于笔者所在医院确诊的82例胃癌患者的癌组织作为观察组,另选取82例健康体检人群正常胃黏膜组织作为对照组。采用酶联免疫吸附法及免疫组化染色法检测PDK-1蛋白在GC组织和正常胃黏膜组织中的表达水平,并分析PDK-1蛋白在胃癌组织中的表达水平与患者各临床病理及PDK-1表达水平与患者预后关系。结果:胃癌组织中的PDK-1表达水平显著性高于正常组织(t=80.36,P<0.01);观察组PDK-1高表达率显著高于对照组;PDK-1蛋白在胃癌组织中的表达水平与胃癌患者性别无相关性(字2=0.001 1,P=0.93),而PDK-1蛋白与胃癌TMN分期、分化程度、有无淋巴结转移呈显著相关性,且TNM分期越高、肿瘤≥35 mm、分化程度越高、有淋巴结转移,PDK-1高表达率越高;PDK-1低表达组5年生存率及生存期均明显高于PDK-1高表达组(字2=8.713,t=16.108,P<0.01)。结论:胃癌组织中PDK-1表达显著性高于正常胃黏膜组织;PDK-1促进了胃癌的发生、发展;PDK-1高表达与胃癌患者的不良预后相关。PDK-1可能作为胃癌的候选基因和潜在的胃癌生物标志物,深入研究PDK-1具有重要的临床意义。

[Abstract] Objective: To study and analyze the expression and clinical significance of PDK-1 protein in gastric cancer, and provide new ideas for the treatment of gastric cancer. Method: The cancer tissues of 82 patients with gastric cancer diagnosed in the hospital from January 2010 to January 2012 were selected as the observation group. The normal gastric mucosa of 82 healthy subjects was selected as the control group. The expression levels of PDK-1 protein in GC tissues and normal gastric mucosa were detected by enzyme-linked immunosorbent assay (ELISA) and immunohistochemical staining. The relationship between the expression level of PDK-1 protein in gastric cancer tissues and clinical pathology and prognosis of patients were analyzed. Result: The expression of PDK-1 in gastric cancer tissues was significantly higher than that in normal tissues (t=80.36, P<0.01). The high expression rate of PDK-1 in the observation group was significantly higher than that in the control group. The expression level of PDK-1 protein in gastric cancer tissues was not correlated with the gender of gastric cancer patients (字2=0.001 1, P=0.93), and PDK-1 protein were significantly correlated with gastric cancer TMN stage, differentiation degree, lymph node metastasis, and the higher the TNM stage, Tumor ≥35 mm, the higher the degree of differentiation, lymph node metastasis, the higher expression rate of PDK-1. The 5-year survival rate and survival time of PDK-1 lower expression group were significantly higher than those of PDK-1 higher expression group (字2=8.713, t=16.108, P<0.01). Conclusion: The expression of PDK-1 in gastric cancer tissues is significantly higher than that in normal gastric mucosa. PDK-1 promotes the occurrence and development of gastric cancer. The high expression of PDK-1 is associated with poor prognosis of gastric cancer patients. PDK-1 may be a candidate gene for gastric cancer and a potential biomarker for gastric cancer. The in-depth study of PDK-1 has important clinical significance.

胃癌是世界上最常見的胃肠道恶性肿瘤,是全球癌症死亡率的第二大原因[1]。在中国每年大约有一半的胃癌发生率和死亡率。在大多数情况下,胃癌已经发展到晚期,并且在诊断时已经导致转移,因此胃癌的5年生存率较低。鉴定新的潜在的胃癌生物标志物和高风险因子,以及阐明胃癌发生、发展的分子机制具有非常重要的临床意义[2-3]。近年来,越来越多研究表明,磷脂酰肌醇依赖性蛋白激酶-1(PDK1)的异常激活与多种癌症的发生和发展密切相关,包括乳腺癌、结肠癌和卵巢癌[4]。PDK-1基因的靶向抑制可显著抑制这些癌症的细胞生长[5]。然而,PDK-1基因的异常表达与胃癌的发生、发展及在癌变中扮演了怎样的角色尚不清楚。笔者研究了胃癌患者肿瘤组织中的PDK-1表达,并分析了PDK-1基因的表达与临床病理特征之间的相关性。结果表明PDK-1可能作为胃癌的候选靶基因和潜在的预后生物标志物,报道如下。……
