红细胞分布宽度及超敏C反应蛋白对急性脑梗死预后的影响

【摘要】 目的:探讨红细胞分布宽度(RDW)及超敏C反应蛋白(hs-CRP)对急性脑梗死预后的影响。方法:将2018年1月-2019年6月笔者所在医院收治的164例急性脑梗死患者作为研究对象,根据改良Rankin量表(modified Rankin scale,mRS)评分标准进行分组,mRS≤2分即为预后良好组(101例),mRS>2分即为预后不良组(63例)。进行单因素分析及多因素Logistic回归分析患者预后的影响因素。结果:单因素分析显示,预后良好组患者NIHSS评分、高脂血症率、糖尿病率、RDW值、hs-CRP水平均低于预后不良组(P<0.05);多因素分析显示,入院NIHSS评分、RDW升高、hs-CRP水平增加均为影响急性脑梗死患者预后的独立危险因素(P<0.05)。结论:急性脑梗死患者RDW及hs-CRP水平对患者预后具有重要的临床价值。

[Abstract] Objective: To investigate the effect of red blood cell distribution width (RDW) and hypersensitive C-reactive protein (hs-CRP) on the prognosis of acute cerebral infarction. Method: A total of 164 patients with acute cerebral infarction admitted in our hospital from January 2018 to June 2019 were selected as the clinical research objects. According to the modified Rankin scale (mRS) score standard, the patients with mRS≤2 points were classified as the group with good prognosis (101 cases), and the patients with mRS>2 points were classified as the group with poor prognosis (63 cases). Univariate analysis and multivariate Logistic regression were used to analyze the influencing factors of prognosis. Result: Univariate analysis showed that NIHSS score, hyperlipidemia rate, diabetes rate, RDW value and hs-CRP level in the group with good prognosis were all lower than those in the group with poor prognosis (P<0.05). Multivariate analysis showed that NIHSS score, increased RDW and increased hs-CRP level were all independent risk factors affecting the prognosis of patients with acute cerebral infarction (P<0.05). Conclusion: RDW and hs-CRP levels in patients with acute cerebral infarction have important clinical value for prognosis.

腦梗死是一种多发性的常见精神系统疾病,且死亡率较高,严重影响患者的正常生活[1]。脑梗死的重要病因之一即为动脉粥样硬化。超敏C反应蛋白(hs-CRP)是动脉粥样硬化和脑梗死的危险影响因素,因此hs-CRP水平升高可作为脑梗死的一个重要标志[2]。随着脑梗死疾病越来越多,临床对此研究也越来越重视,通过研究发现红细胞分布宽度(RDW)对脑梗死疾病有一定的诊断和预测价值[3]。RDW是通过运用全自动血细胞分析仪测得反映出红细胞体积变异的指标,通常运用在贫血的诊断中[4]。笔者所在医院为研究RDW和hs-CRP对急性脑梗死预后的影响,特选取164例急性脑梗死患者作为研究对象,选取时间为2018年1月-2019年6月,现将结果报道如下。

1 资料与方法

1.1 一般资料

将2018年1月-2019年6月笔者所在医院收治的164例急性脑梗死患者作为临床研究对象,纳入标准:(1)符合脑梗死诊断标准,并经过临床检查确诊;(2)发病时间<72 h;(3)入院时患者出现局灶性神经性功能缺失[5]。排除标准:(1)曾做过溶栓治疗;(2)复发性脑梗死;(3)入院前有感染病史;(4)患有恶性肿瘤;(5)3个月前曾有输血史[6]。根据改良Rankin量表(modified Rankin scale,mRS)评分标准分组,mRS≤2分即为预后良好组(101例);mRS>2分即为预后不良组(63例)。按照……
