【摘要】 目的：观察根除幽门螺杆菌治疗对类风湿关节炎幽门螺杆菌阳性患者血清IL-8、IL-18水平的影响。方法：以2017年12月前未接受根除幽门螺杆菌治疗的类风湿关节炎幽门螺杆菌阳性患者作为对照组，接受根除幽门螺杆菌治疗的类风湿关节炎幽门螺杆菌阳性患者作为观察组，对照组给予艾拉莫德+甲氨蝶呤治疗，观察组在对照组的基础上，给予雷贝拉唑+克拉霉素+阿莫西林治疗。结果：观察组治疗后CRP（3.00±0.59）mg/L，IL-8（35.20±1.63）ng/L，IL-18（40.30±2.30）pg/ml，晨僵时间（10.65±0.14）min，关节握力（7.42±1.00）kPa，关节肿胀（1.20±0.02）个，关节疼痛（1.03±0.36）分，治疗有效率95%，优于对照组，差异有统计学意义（P<0.05）。结论：根除幽门螺杆菌，有助于调节类风湿关节炎幽门螺杆菌阳性患者血清IL-8、IL-18水平，减轻炎性反应。

[Absract] Objective： To observe the effect of eradication of Hp on serum IL-8， IL-18 level in patients with positive Hp in rheumatoid arthritis. Method： The patients with positive HP who had not been treated with Hp eradication before December 2017 were used as the control group. The patients with positive Hp who had been treated with Hp eradication were selected as the observation group. The control group was treated with Iguratimod， Methotrexate， and the observation group was treated with Rabeprazole， Clarithromycin， Amoxicillin on the basis of the control group. Result： After treatment， CRP was （3.00±0.59） mg/L， IL-8 （35.20±1.63） ng/L， IL-18 （40.30±2.30） pg/ml， morning stiffness time （10.65±0.14） min， joint grip strength （7.42±1.00） kPa， joint swelling （1.20±0.02）， joint pain （1.03±0.36） points， the effective rate was 95% in the observation group， which were significantly better than those in the control group （P<0.05）. Conclusion： Eradication of Hp is helpful to regulate serum IL-8， IL-18 level and reduce inflammatory reaction in patients with rheumatoid arthritis with positive Hp.

類风湿关节炎是以慢性炎性滑膜炎为主要特征的系统性疾病，多发生于手、足等关节，病因尚未明确[1]。有研究指出，幽门螺杆菌感染可能通过调节血清IL-8、IL-18水平等途径，参与到自身免疫性疾病的发生与进展中[2]。同样有报道显示，与健康人群相比，类风湿关节炎患者，幽门螺杆菌感染率更高，而患者可自根除幽门螺杆菌中获益[3]。为提高类风湿关节炎的治疗有效率，明确疾病与幽门螺杆菌感染之间的关系，本文于笔者所在医院2017年12月-2018年12月收治的类风湿关节炎幽门螺杆菌阳性患者中，选取其中进行抗幽门螺杆菌治疗的20例作为观察组，回顾性分析了根除幽门螺杆菌治疗对患者血清IL-8、IL-18水平的影响，报告如下。
