摘 要：我科于2017年10月收治1例马尾综合征伴泌尿系多重耐药菌感染及急性肾功能损伤患者，该患者于住院期间又发生铜绿假单胞菌二重感染，经我科护理组对该患者密切观察和精心护理，特别是泌尿系统方面的管理及指导，成功为患者解除病痛。对于该类患者，其泌尿系统的护理极其重要，与预后密切相关，所以，我们在关注神经功能恢复的同时，也要注意对该类患者泌尿系统的护理。

Abstract：In October 2017，our department admitted to a patient with cauda equina syndrome with urinary multi-drug resistant infection and acute renal function injury.This patient developed a double infection of Pseudomonas aeruginosa during hospitalization， which was followed by our nursing group.The patient closely observed and meticulous care，especially the management and guidance of the urinary system，successfully relieved the patient from the pain.For this type of patients，the nursing of the urinary system is extremely important and closely related to the prognosis.Therefore，we should pay attention to the nursing of the urinary system of this type of patients while paying attention to the recovery of neurological function.

馬尾综合征（cayda equina syndrome，CES）为腰骶神经受压所致的腰腿疼痛、下肢无力、鞍区感觉障碍及肛门、膀胱括约肌及性功能障碍的一种症候群[1]。马尾综合征常常合并神经源性膀胱，而尿液潴留、大便失禁等又可导致泌尿系感染，严重者可发展为急性肾盂肾炎，导致急性肾功能损伤，危及生命。我科于2017年10月收治1例马尾综合征伴泌尿系多重耐药菌感染及急性肾功能损伤患者，给予严密的观察和护理后一般状况得到极大改善，现报告如下。

1病例简介

患者，男性，69岁，因“二便失禁1年”入院。查体：T 36.6 ℃，P 80次/min，R 18次/min，BP 105/65 mmHg。右足背浅感觉消失，双侧髂腰肌肌力4级，股四头肌、胫前肌、踇长伸肌和小腿三头肌5级。双下肢肌张力正常。会阴部浅感觉消失，肛门括约肌张力消失。阴茎包皮嵌顿、水肿伴溃烂，直腿抬高试验（-），4字实验（-），病理征（-）。脊柱MRI：L2～3椎体层面椎管内髓外硬膜外占位性病变，脊髓圆锥、马尾受压，考虑为脂肪瘤。……
