[摘要] 目的 目的探究心脏超声对高血压左室肥厚伴左心力衰竭的临床诊断价值。 方法 方便选取2014年6月—2017年6月在该院行心脏超声检查的54例高血压左室肥厚伴左心力衰竭患者作为观察组，另选择同期健康体检的47例健康者作为对照组。统计并比较两组左室舒张末期内径（LVDd）、左房内径（LAD）、左室射血分数（LVEF）和早期峰值流速 / 舒张晚期峰值流速（E/Ea）等指标水平，并对比分析不同心功能分级患者各指标水平。 结果 观察组LVDd（56.37±7.18）mm、LAD（43.67±6.79）mm、E/Ea（12.71±3.03）%指标水平均显著高于对照组（43.67±6.57）mm、（27.64±3.69）mm、（6.89±2.14）%，观察组LVEF指标（51.69±8.43）%显著低于对照组（70.41±7.02）%，组间差异有统计学意义（P<0.05）。心功能Ⅰ级[（47.67±7.14）mm、（42.31±7.15）mm、（60.37±6.79）%、（10.57±5.33）%]与Ⅱ级、Ⅲ级患者比较，LVDd、LAD、LVEF、E/Ea各指标水平差异有统计学意义（P<0.05）。心功能Ⅱ级（53.24±6.89）mm、（51.34±8.57）mm、（53.74±7.13）%、（12.69±4.37）%与Ⅲ级（70.35±7.83）mm、（60.34±7.54）mm、（46.37±6.98）%、（18.16±6.58）%患者比较，LVDd、LAD、LVEF、E/Ea各指標水平均差异有统计学意义（P<0.05）。结论 心脏超声对高血压左室肥厚伴左心力衰竭诊断价值较高，可为该病临床诊断、治疗提供可靠依据。

Echocardiographic Diagnosis of Hypertensive Left Ventricular Hypertrophy Associated with Left Heart Failure

[Abstract] Objective This paper tries to investigate the clinical value of echocardiography in the diagnosis of hypertensive left ventricular hypertrophy and left heart failure. Methods 54 patients with left ventricular hypertrophy and left heart failure in this hospital from June 2014 to June 2017 were convenient selected by echocardiography， and 47 healthy subjects were selected as control group at the same time in the same period. The left ventricular end diastolic diameter （LVDd）， left atrial diameter （LAD）， left ventricular ejection fraction （LVEF） and the peak velocity of the early / late diastolic peak velocity （E/Ea） and other indicators were concluded and compared between the two groups， and the indexes of heart function in patients were compared. Results The observation group， LVDd （56.37±7.18）mm， LAD （43.67±6.79）mm， E/Ea （12.71± 3.03）%， were significantly higher than the control group of（43.67± 6.57）mm，（27.64±3.69）mm，（6.89±2.14）%， the observation group LVEF index （51.69±8.43%） was significantly lower than the control group （70.41±7.02）%， between groups the difference was significant （P<0.05）. Cardiac function class I （47.67±7.14）mm， （42.31±7.15）mm， （60.37±6.79）%， （10.57±5.33）% was compared with patients of grade II and III. There were significant differences in the levels of LVDd， LAD， LVEF and E/Ea（P<0.05）. Heart function grade II （53.24±6.89）mm， （51.34±8.57）mm，（53.74±7.13）%，（12.69±4.37）% and III （70.35±7.83）mm， （60.34±7.54）mm， （46.37± 6.98）%， （18.16±6.58）%， LVDd， LAD， LVEF， E/Ea indicators showed significant differences（P<0.05）. Conclusion The diagnostic value of echocardiography in hypertensive left ventricular hypertrophy with left heart failure is high， and it can provide reliable basis for clinical diagnosis and treatment of this disease.

高血压为我国第一大慢性病，而心血管并发症是该群体最常见的并发症，高血压患者心血管疾病发病率较高[1]。高血压左室肥厚伴左心力衰竭为老年人群的常见病，早期准确准确、及时干预，可有效延缓病程进展，降低患者死亡率[2]。心脏超声检查是心力衰竭临床诊断的重要手段，为进一步探明其在高血压左室肥厚伴左心力衰竭临床诊断方面的应用价值，该次研究方便选取2014年6月—2017年6月期间在该院行心脏超声检查的54例高血压左室肥厚伴左心力衰竭患者和47例健康者作为研究对象，对比分析了其诊断效果，现报道如下。
