[摘要] 目的 討论分析在临床工作中诊断癫痫时应用长程视频脑电监测（VEEG）的效果。方法 方便选择的研究对象是2016年1月—2017年10月期间就诊的可疑癫痫患者100例，其中有20例拟诊为非癫痫发作性疾病，80例拟诊为癫痫，给予100例患者分别进行常规脑电图检查和长程视频脑电图检查，对检测出的结果进行分析。结果 80拟诊为癫痫发作患者检查的结果显示。长程视频脑电监测到癫痫样波的有64例患者，癫痫样波的检出率是80.0%，常规脑电图检测到癫痫样波的有40例患者，癫痫样波的检出率是50.0%，常规脑电图检查癫痫样波的检出率明显比长程视频脑电监测对癫痫样波的检出率低。两组之间差异有统计学意义（P<0.05）。20例拟诊为非癫痫发作性疾病患者检查的结果显示。长程视频脑电监测有1次以上临床发作患者20例，发作期和发作间期都没有出现癫痫样放电现象的患者有18例（90.0%），与临床表现相结合，癫痫的可能性被排除。结论 长程视频脑电监测与普通脑电图相比，可持续监测，患者发作时的临床表现可从录像观察。把患者的临床发作与脑电图有效的联系起来，癫痫样异常放电的检出率得到提高，癫痫的检出率得到提高。有助于癫痫的诊断及临床分型，被推广应用。

Application Effect of Long Range Video EEG Monitoring in the Diagnosis of Epilepsy

[Abstract] Objective This paper tries to discuss and analyze the effect of long-range video-electroencephalography （VEEG） monitoring in the diagnosis of epilepsy in clinical settings. Methods The subjects selected were 100 suspicious epilepsy patients from January 2016 to October 2017， of whom 20 were diagnosed as non-seizure disease and 80 were diagnosed as epilepsy and 100 patients were treated separately by routine electroencephalography and long-range video EEG examination， the results were analyzed. Results 80 diagnosed as seizures in patients with the test results showed. Long-range video EEG epilepsy was detected in 64 patients， the detection rate of epileptic-like wave is 80.0%， conventional electroencephalography detected epileptiform wave of 40 patients， the detection rate of epileptic-like waves was 50.0%， the detection rate of epileptiform waves by conventional electroencephalography was significantly lower than the detection rate of epileptiform waves by long-range video EEG. The difference between the two groups was statistically significant （P<0.05）. Twenty patients who were diagnosed as having non-seizure disease were examined. There were 20 patients with long-term video EEG monitoring for more than one clinical attack， 18 patients（90.0%） having no epileptiform discharge during the attack and interictal periods， and the possibility of epilepsy was excluded in combination with clinical manifestations. Conclusion Long-range video EEG monitoring compared with normal EEG， sustainable monitoring， clinical manifestations of patients with seizures can be observed from the video. The patients clinical seizures and EEG effectively linked to the detection rate of epileptiform abnormal discharge is improved， the detection rate of epilepsy is increased， which helps the diagnosis and clinical classification of epilepsy， so it is worth promotion and application.

神经系统的疾病中癫痫是常见的，癫痫的诊断是临床上通过观察患者的临床表现，做脑电图看有无癫痫样放电，只靠脑电图的单次检测出现假阴性比较多，癫痫样放电检出比较少。长程视频脑电监测的检出率高于常规脑电图，在原来常规脑电图的基础上与同步的录像技术相结合，把患者的脑电波变化情况和发作时的临床表现长时间的记录下来，通过后来的回放进行分析，检出率和诊断水平得到提高。常规脑电图只能短时间记录，检出率不高，容易发生漏诊[1]。随着新技术的发展长程视频脑电监测已经广泛应用于临床，具有对患者脑电图检测时间长而不间断的优点，提高癫痫的检出率[2]。……
