根因分析在内科住院患者跌倒风险管理中的应用分析

摘 要：目的 分析住院患者发生跌倒的原因，探讨根因分析（RCA）在住院患者跌倒中的应用效果。方法 选取我院2016年1月～12月的38起跌倒事件，采用RCA方法分析近端原因、根本原因，通过改变环境、优化防跌倒流程、加强人员培训等措施预防跌倒发生，比较实施前后患者跌倒发生率。结果 实施后，跌倒发生率为0.14%，低于实施前的0.38%，差异具有统计学意义（P<0.05）。结论 RCA应用于内科住院患者跌倒风险管理，对预防住院患者跌倒具有很好的导向作用。

Analysis of the Application of Root Cause Analysis in Fall Risk Management of Inpatients in Internal Medicine

Abstract：Objective To analyze the causes of falls in hospitalized patients and to explore the effect of root cause analysis（RCA） on falls in hospitalized patients.Methods 38 fall events from January to December 2016 were selected. The RCA method was used to analyze the causes and root causes. The measures were taken to change the environment， optimize the fall prevention process， and strengthen personnel training to prevent falls.Compare the incidence of falls before and after implementation.Results After the implementation， the incidence of falls was 0.14%， which was lower than 0.38% before the implementation，the difference was statistically significant（P<0.05）. Conclusion RCA is applied to the management of fall risk in medical inpatients and has a good guiding effect on preventing falls in hospitalized patients.

跌倒是指突發、不自主、非故意的导致身体任何部位（不包括双脚）的体位改变，倒在地面或比初始位置更低的平面上。跌倒不仅使患者身体受到伤害，延长住院时间，增加家庭和社会的负担，而且也会增加患者及家属对医院的不信任感[1]。根因分析（root cause analysis，RCA）是一种解决问题的方法，其旨在定位问题根本原因并最终使问题得到解决。我科利用RCA法分析内科住院患者发生跌倒的原因，采取相应措施，减少跌倒发生率，提高了患者的满意度，现报道如下。
1资料与方法
1.1一般资料 选取2016年1月～12月重庆医科大学附属第一医院内科病房发生的38例跌倒患者，本研究经医院伦理会批准。其中男22例，女16例，年龄50～85岁，平均年龄（60.00±7.51）岁 ;文化程度：小学及以下5例，初中及中专15例，高中及以上18例;发生时间：8：00～16：00发生7例，16：00～4：00发生14例，0：00～8：00发生17例;发生地点：床边11例，厕所19例，走廊6例，楼梯2例;有陪护22例，无陪护16例。
