[摘要]目的：观察眼轮匝肌瓣联合自体脂肪填充治疗上睑凹陷的效果。方法：自2012年以来，对22例上睑凹陷患者采用眼轮匝肌瓣联合自体脂肪填充的方法进行治疗。结果：本组22例患者，除1例上睑凹陷纠正不足行第二次脂肪移植注射外，其余患者自觉效果满意。结论：采用眼轮匝肌瓣联合自体脂肪填充是治疗治疗上睑凹陷的有效方法。

Correction of Sunken Upper Eyelid with Orbicularis Oculi Muscle Flap Combined with Autologous Fat Transplantion

Abstract： Objective To observe the effects of correction of sunken upper eyelid with orbicularis oculi muscle flap combined with autologous fat transplantion. Methods 22 patients with Sunken upper eyelid were enrolled in this study since 2012. All the patients were treated with orbicularis oculi muscle flap combined with autologous fat transplantion. Results One patient was given a second time autologous fat injection to further recompense the Sunken. All the rest 21 patients all reached satisfied clinical effects after the treatments. Conclusion Orbicularis oculi muscle flap combined with autologous fat transplantion is an effective technique to reconstruct sunken upper eyelid.

上瞼凹陷是指上睑板上方与眶上缘之间的上睑出现不同程度的向内陷入的状态[1]。临床一般依据病因将上睑凹陷分为先天性上睑凹陷和后天性上睑凹陷[2]。东方人上睑凹陷常有多重睑，感觉人疲惫、无神、衰老。矫正上睑凹陷的方法较多，大多采用自体颗粒脂肪填充，但因自体脂肪成活率不稳定，效果一般。自2012年以来，笔者采用眼轮匝肌瓣联合自体脂肪填充的方法，治疗了22例患者，效果满意。

1 临床资料

本组共22例生理性上睑凹陷患者，均为女性，年龄26～50岁，平均35岁，均为双侧。16例伴有内眦赘皮。

2 手术方法

2.1 术前设计：坐位下观察上睑形态，标记上睑凹陷范围。卧位下根据患者情况按重睑切口设计切口线，内眦赘皮按Z成形术设计。在取脂区标记范围，首选下腹部[3]，切口设计在脐下方，如腹部有吸脂史则选择在大腿外侧取脂肪，吸脂口设计在臀沟外侧。

2.2 在上睑部皮下用2%利多卡因（内含1：20万肾上腺素）行局部浸润麻醉，按设计线切开皮肤，将多余皮肤切除，在切口下缘向下锐性分离皮肤和眼轮匝肌，至距睑缘2mm处，切开眼轮匝肌至睑板前筋膜组织，在眼轮匝肌深面锐性分离至睑板上缘。……
