脑梗死患者脂蛋白相关磷脂酶A2(LP—PLA2)水平与血管性认知障碍(VCI)的相关研究

[摘要] 目的 探讨脑梗死患者脂蛋白相关磷脂酶A2(LP-PLA2)水平与血管性认知障碍(VCI)的相关性。方法 便利选取2016年1月—2017年5月该院收治的90例脑梗死患者为研究对象,全部患者均在病程3 d内抽取静脉血检测血清LP-PLA2水平,依据检测结果将入选病例分为实验组(LP-PLA2>175 ng/mL,45例)和对照组(LP-PLA2≤175 ng/mL,45例)。治疗后随访,以蒙特利尔认知量表(MoCA)评价两组认知功能,统计两组VCI发病率。结果 实验组MoCA评分(22.1±2.7)分,VCI发病率57.8%,与对照组比较差异有统计学意义(P<0.05)。结论 LP-PLA2高水平可能是脑梗死患者VCI不良预后的临床标志物,加强LP-PLA2观察对VCI临床诊断具有一定参考价值。

Correlation between Lipoprotein-related Phospholipase A2 (LP-PLA2) Level and Vascular Cognitive Impairment (VCI) in Patients with Cerebral Infarction

[Abstracts] Objective This paper tries to investigate the correlation between lipoprotein-related phospholipase A2 (LP-PLA2) level and vascular cognitive impairment (VCI) in patients with cerebral infarction. Methods Convenient selection 90 patients with cerebral infarction from January 2016 to May 2017 were selected and extracted venous blood serum LP-PLA2 level within 3 days of course, and were divided into the experimental group(LP-PLA2>175 ng/mL, 45 cases) and the control group(LP-PLA2≤175 ng/mL, 45 cases) according to the test results. The cognitive function of the two groups was evaluated by the Montreal Cognitive scale(MoCA) after the treatment, and the incidence of VCI of the two groups was statistically analyzed. Results The MOCA score was(22.1±2.7)points and the incidence of VCI was 57.8% in the experimental group, which was statistically significant compared with the control group.(P<0.05). Conclusion LP-PLA2 high level may be a clinical marker of VCI prognosis in patients with cerebral infarction, and strengthening LP-PLA2 observation has certain reference value for VCI clinical diagnosis.

脑梗死是脑局部血氧供应障碍所引起的脑组织变性坏死及相关神经功能缺失,又称缺血性卒中,属临床常见病、多发病,致残、致死率高,对人类生命健康威胁极大[1]。LP-PLA2为非钙离子依赖型磷酸酯酶,有血管特异性,是近年临床研究已经证实的缺血性脑卒中炎症反应因子和独立危险因素[2]。VCI是血管危险因素、明显或不明显脑血管病引起的,以不同程度认知功能障碍为表现的临床综合征,主要致病因素包括三高症、脑梗死、脑出血、慢性脑缺血等。文章现便利选取2016年1月—2017年5月该院收治的90例脑梗死患者为例,对 LP-PLA2与VCI间的关系进行研究,现报道如下。
1 资料与方法
1.1 一般资料
便利选取该院收治的90例脑梗死患者为研究对象,依LP-PLA2水平将入选病例分为两组。实验组(45例):男25例,女20例;年龄39~87岁,平均(61.1±5.7)岁;小学及以下文化7例,初高中文化30例,大专及以上文化8例;入院时GCS评分7~13分,平均(10.9±2.2)分。梗死灶体积:5 cm3以下27例,5~10 cm3 11例,10 cm3以上7例。对照……
