[摘要] 目的 分析我院药品不良反应ADR出现漏报情况，将会对患者造成伤害，需要针对其进行全面有效的控制和管理，以此降低药品不良反应的发生率、促进临床合理用药。 方法 采用回顾性调查研究方法对我院2015～2016年上报ADR 124例和漏报的ADR 230例进行统计分析。 结果 漏报的230例ADR报告中以静脉给药引发的ADR最多，其中又以调节免疫药物方面引发的ADR最多，达到41.28%。门诊耳鼻喉科ADR的漏报率最高，达到84.88%。 结论 临床应加强ADR的监测，如实上报ADR的发生率，避免或减少药品不良反应发生。

Investigation and causes analysis of adverse drug reactions in our hospital

[Abstract] Objective To explore the adverse drug reactions （ADR） missing report in our hospital， which would cause harm to the patient. Comprehensive and effective control and management was required. In order to reduce the incidence of adverse drug reactions， and to promote rational use of drugs in clinical practice. Methods Retrospective investigation and research method was used to statistically analyze the 124 cases report ADR and 230 cases of missed report ADR in our hospital in 2015 -2016. Results Among the 230 cases of missing report ADR， the most was ADR caused by intravenous administration. Among them， ADR caused the regulation of immune drugs was the most， up to 41.28%. The outpatient department of ENT， had the highest false negative rate of ADR， reaching 84.88%. Conclusion We should strengthen the monitoring of ADR and truthfully report the incidence of ADR to avoid or reduce adverse drug reactions.

药品不良反应（adverse drug reactions），主要是指合格药品在正常用法用量下出现的与用药目的无关或意外的有害反应。药品不良反应是客观存在的，既不是医疗事故，也不是质量事故。药品不良反应报告和监测是指药品不良反应的发现、报告、评价和控制的过程。药品不良反应上报原则：可疑即报。医院是使用药品的主要场所，也是我国ADR的主要报告部门，因此，加强医院的ADR监测至关重要，其对促进临床合理用药、保障患者用药安全具有重要意义。但是，现阶段仍然有一些医院存在着药品不良反应漏报的情况，需要积极针对其进行全面调查，查找分析其中的原因，提出行之有效的措施，提高医护人员上报ADR的积极性，增加上报率，提高ADR报告的质量。
