【摘要】 目的 探討经骨钢丝固定导引器在膝关节前交叉韧带（ACL）胫骨止点撕脱性骨折钢丝内固定术中的应用效果。方法 23例膝关节前交叉韧带胫骨止点撕脱性骨折患者， 随机分为实验组（12例）和对照组（11例）。实验组应用经骨钢丝固定导引器进行钢丝内固定手术， 对照组采用传统切开手术方法进行钢丝内固定术。观察两组治疗效果。结果 所有患者无关节感染， 随访期间内未发现创伤性关节炎发生。观察组手术切口、术中出血量小于对照组， 手术时间短于对照组， 差异有统计学意义（P<0.05）。观察组术后并发症发生率为8.33%， 低于对照组的54.55%， 差异有统计学意义（P<0.05）。观察组术后愈合时间、Lysholm评分、国际膝关节文献委员会膝关节评估表（IKDC）评分及优良率均优于对照组， 差异有统计学意义（P<0.05）。结论 在膝关节前交叉韧带胫骨止点撕脱性骨折钢丝固定手术中使用经骨钢丝固定导引器效果良好， 可以显著缩短手术时间、减少术中失血量， 减小损伤， 可达到骨折良好的复位及稳定固定的效果， 利于骨折愈合及术后恢复， 降低并发症发生率， 效果显著， 值得临床推广。

【Abstract】 Objective To explore the application effect of bone wire fixed guidance in wire internal fixation for tibial avulsion fracture of anterior cruciate ligament （ACL）. Methods A total of 23 patients with tibial avulsion fracture of anterior cruciate ligament were randomly divided into experimental group （12 cases） and control group （11 cases）. The experimental group received bone wire fixed guidance for wire internal fixation， and the control group received traditional open surgery for wire internal fixation. Therapeutic effect was observed in two groups. Results All patients had no joint injection， and no cases of traumatic arthritis appeared during follow-up. The observation group had less surgical incision and intraoperative bleeding volume than the control group， and shorter operation time than the control group. Their difference had statistical significance （P<0.05）. The observation group had lower incidence of postoperative complications as 8.33% than 54.55% in the control group， and the difference had statistical significance （P<0.05）. The observation group had better postoperative healing time， Lysholm score， International Knee Document Committee （IKDC） score and excellent-good rate than the control group， and their difference had statistical significance （P<0.05）. Conclusion Bone wire fixed guidance shows good effect in wire internal fixation for tibial avulsion fracture of anterior cruciate ligament， which can significantly shorten operation time， reduce intraoperative bleeding volume and injury， achieve good effect for fracture reduction and stable fixation， help fracture healing and postoperative recovery and decrease incidence of complications. So it is worthy of clinical promotion.

随着社会发展， 目前因运动及交通事故引起的膝关节损伤逐年增加， 其中前交叉韧带胫骨止点撕脱性骨折是急性膝关节损伤中一种常见疾病， 多由于高处坠落、跳跃、运动或车祸时膝关节过伸或胫骨旋转超过前交叉韧带负荷时， 可发生胫骨撕脱性骨折[1]， 在儿童和青少年膝关节外伤中约占2%～5%[2]， 成年人胫骨棘撕脱骨折的发病率不断增高， 占前交叉韧带损伤的14%[3， 4]。对于有移位的撕脱性骨折， 非手术治疗效果不理想， 经常发生膝关节功能恢复不佳、膝关节不稳等不良后果， 严重者会导致创伤性关节炎的发生， 对患者的生活及工作产生严重影响。对于该种疾病的治疗， 临床上期望目标是膝关节稳定、功能良好、并发症少， 采用手术内固定治疗已被广泛接受， 通过坚强内固定， 可避免出现膝关节不稳等并发症[5]。……
