艾塞那肽治疗2型糖尿病疗效的Meta分析

[摘要] 目的 评价艾塞那肽治疗2型糖尿病的疗效。 方法 计算机检索PubMed、Cochrane、万方数据库、CNKI数据库,收集关于艾塞那肽治疗2型糖尿病的随机对照试验(RCT),对纳入的文献进行质量评价并提取相关资料。应用RevMan软件进行Meta分析。 结果 共纳入11个RCT。Meta分析结果显示:在疗效方面,与安慰剂组或空白对照组相比,艾塞那肽能显著降低2型糖尿病患者的糖化血红蛋白及空腹血糖[MD=-1.15,95%CI(-1.19,-1.11),P<0.000 01;MD=-1.34,95%CI(-1.46,-1.22),P<0.000 01]。 结论 艾塞那肽在降低2型糖尿病患者血糖方面有显著疗效。

Curative Effect of Exenatide in Treatment of Type 2 Diabetes and Meta Analysis

[Abstract] Objective To evaluate the curative effect of exenatide in treatment of type 2 diabetes. Methods The PubMed,Cochrane, wanfang database and CNKI database were retrieved by the computer, and the RCT of exenatide in treatment of type 2 diabetes was collected and the included literatures were given the quality evaluation and the related data were extracted and the Meta analysis was conducted by using the RevMan software. Results 11 RCT were included, and the Meta analysis results showed that the exenatide can obviously reduce the glycosylated hemoglobin and fasting blood glucose of patients with type 2 diabetes compared with that in the placebo group or blank control group[MD=-1.15,95%CI(-1.19,-1.11), P<0.00001; MD=-1.34,95%CI(-1.46,-1.22), P<0.000 01]. Conclusion The curative effect of exenatide in reducing the blood glucose of patients with type 2 diabetes.

人胰高血糖样肽-1(GLP-1)是由空肠末端、回肠、结肠的L细胞在摄食和神经内分泌刺激下分泌的一种具有葡萄糖浓度依赖性的肠促胰岛素释放肽。GLP-1具有促胰岛素分泌的生理效应,且其作用呈葡萄糖浓度依赖性,随着血糖升高,胰岛素分泌量增加。GLP-1还有增加胰岛素合成、抑制胰高血糖素分泌、刺激胰岛β细胞增殖分化、抑制食欲等生理效应。GLP-1的生物学特性奠定了其在糖尿病新治疗方案中的地位[1]。艾塞那肽是首个获准的GLP-1类似物药物,现已逐渐进入国内各大医院,成为中国2型糖尿病患者新的治疗手段。目前国内对于艾塞那肽治疗2型糖尿病患者疗效方面的系统评价较少,艾塞那肽长期应用的疗效还有待进一步评价。故该文拟采用Meta分析方法,评价艾塞那肽治疗2型糖尿病的疗效,为临床用药提供参考,现报道如下。
1 资料与方法
