2型糖尿病慢性并发症与空腹C肽、HbA1c及果糖胺相关性的研究

[摘要] 目的 探讨空腹C肽、糖化血红蛋白(HbA1c)、果糖胺(FRA)与2型糖尿病(T2DM)慢性并发症的关系。方法 选取121例糖耐量减低(IGT)患者作为对照组,116例单纯T2DM患者及271例T2DM慢性并发症组作为病例组,比较各组血糖、C肽、HbA1c及FRA等指标。结果 单纯T2DM组、T2DM慢性并发症组各时间点的PG较IGT组明显升高(P<0.05);单纯T2DM组、T2DM慢性并发症组各时间点的C肽较IGT组明显减低(P<0.05)。单纯T2DM组、T2DM慢性并发症组的HbA1c、FRA较IGT组明显升高(P<0.05), T2DM慢性并发症组较单纯T2DM组的HbA1c、FRA明显减少(P<0.05)。Spearman 相关分析发现空腹C肽与HbA1c、FRA呈负相关(P<0.05)。结论 空腹C肽、HbA1c、FRA与2型糖尿病慢性并发症发生密切相关,降低HbA1c、FRA及血脂对防治T2DM慢性并发症有重要意义。

Research on Correlation between the Chronic Complications of Type 2 Diabetes and Fasting C-peptide, HbA1c and Fructosamine

[Abstract] Objective To study the correlation between the chronic complications of type 2 diabetes and fasting C-peptide, HbA1c and fructosamine. Methods 121 cases of IGT patients were selected as the control group, 116 cases of simple T2DM patients and 271 cases of patients with T2DM chronic complications were selected as the case group, and the blood glucose, C-peptide, HbA1c and FRA indexes were compared between the two groups. Results The PG in various time points in the simple T2DM group and T2DM chronic complication group obviously increased compared with that in IGT group(P<0.05), and the C-peptide in the simple T2DM group and T2DM chronic complication group obviously increased compared with that in the IGT group(P<0.05), and the HbA1c and FRA in the T2DM chronic complication group obviously decreased compared with those in the HbA1c and FRA groups(P<0.05), and the Spearman related analysis showed that the fasting C-peptide was negatively correlated with the HbA1c and FRA(P<0.05). Conclusion The correlation between the fasting C-peptide, HbA1c, FRA and type 2 diabetes chronic complications is close, and decreasing the HbA1c, FRA and blood liquid is of important significance to the prevention of T2DM chronic complication.

目前2型糖尿病(T2DM)是全球性的健康问题, 据国际糖尿病联合会预计,世界上糖尿病患者从2016年的4亿多患病人数到2040年可能超过6.4亿人数[1]。T2DM的病程、血糖控制状态、血脂水平等是慢性并发症发病(如T2DM肾病、周围神经病变、视网膜病变等)的主要相关因素,其慢性并发症控制不佳将增加患者死亡率[2]。国內外研究发现葡萄糖耐受异常、C肽分泌异常、HbA1c增高引起的代谢变化可导致T2DM慢性并发症[2-4],而果糖胺(FRA)在血糖监测体系中是对HbA1c有效补充[5]。该研究分析了空腹C肽、HbA1c及FRA与T2DM慢性并发症的相关性,旨在探讨该3个检测项目在慢性并发症发生发展中的影响及为其防治提供重要的理论依据,现报道如下。
1 资料与方法
1.1 一般资料
选取2015 年1月—2016年12月间于该院内分泌科门诊或住院就诊的121例糖耐量减低(IGT)患者作为对照,116例单纯T2DM患者及271例T2DM慢性并发症组(其中60例T2DM肾病患者,60例T2DM视网膜病变,60例T2DM周围神经病变患者,91例T2DM多发并发症患者)。纳入标准:①T2DM诊断依据WHO糖尿病专家委员会制定的糖尿病诊断标准(1999年)[6]。②T2DM慢性并发症患者符合中国2型糖尿病防治指南(2013年版)[2]中关于T2DM肾病、视网膜病变、周围神经病变等慢性并发症诊断标准。……
