【摘要】目的了解青年乳腺癌患者术后化疗期抑郁的发生状况，探讨治疗过程中抑郁的影响因素。

方法采用自行设计的一般资料问卷、M.D.Anderson症状调查量表（MDASI）和抑郁自评量表（SDS）对150例患者在术后化疗期间进行问卷调查。

结果有66%的患者在术后化疗期存在不同程度的抑郁障碍，其抑郁水平高于国内常模（P<0.001）。多因素分析结果显示，家庭关系、对病情的主观感受、疾病造成的经济负担是青年乳腺癌患者术后化疗期抑郁的主要影响因素（P<0.001）。在化疗导致的不良反应中，恶心、疲劳、呕吐、睡眠不安是导致患者抑郁的主要影响因素（P<0.001）。

结论术后化疗期是青年乳腺癌患者发生抑郁的易感时期，医护人员可根据主要影响因素制定相应的预防护理措施，以降低患者的抑郁水平。

【Abstract】ObjectiveTo investigate incidence of depression in young patients with breast cancer during postoperative chemotherapy period，so as to analyze influencing factors of depression during the course of treatment.

Methods150 patients who underwent chemotherapy after operation were investigated by selfdesigned general information questionnaire，M.D.Anderson Symptom Inventory（MDASI） and Selfrating Depression Scale（SDS）.

Results66% of patients have depressive disorder to various levels，and the level of depression was higher than the domestic norm（P<0.001）.Multiple regression analysis showed that family relationship，subjective feeling to the disease，economic burden caused by disease were the main influencing factors of depression（P<0.001）.In chemotherapy side effects，nausea，fatigue，vomiting，sleep disturbances were the main influencing factors of depression（P<0.001）.

ConclusionPostoperative chemotherapy is a susceptible period of depression in young patients with breast cancer，medical staff should pay more attention to the main influencing factors and develop appropriate preventive measures to reduce the level of depression.

我国女性乳腺癌发病率逐年上升，并且日趋年轻化，每年因乳腺癌死亡的女性达1.3万[1]。Hopko等[2]研究发现，重度抑郁症是乳腺癌患者最常见的精神疾病。青年乳腺癌患者抑郁的患病率显著高于老年患者，在35岁以下的患者中，抑郁的患病率高达80%[3]。目前，乳腺癌最主要的治疗方式是手术和化疗，术后化疗期是治疗的关键时期。本研究通过对150例青年乳腺癌患者术后化疗期抑郁状况进行调查，分析其影响因素，为临床治疗护理提供依据，现报告如下。

1对象与方法

1.1调查对象
