【摘 要】目的：分析首次剖腹产切口的选择对二次剖腹产手术的影响。方法：回顾性分析2016年1月到2016年12月在本院实施二次剖腹产的76例产妇资料，基于产妇首次剖腹产所选择切口方式，划分为横切组（腹壁横切口）与纵切组（腹壁纵切口），每组各38例产妇。比较两组产妇的开腹时间、胎儿娩出时间、术中出血量以及患者的术后腹壁粘连程度对比。结果：纵切组产妇的开腹时间、胎儿娩出时间以及术中出血量均低于横切组产妇，各项数据对比差异显著（p<0.05）。纵切组产妇术后粘连程度显著优于横切组产妇，差异对比具有统计学意义（p<0.05）。结论：首次剖腹产切口选择腹壁纵切口，效果显著优于选择腹壁横切口，能够缩短产妇开腹的时间与胎儿娩出时间，减少产妇的术中出血量，且有助于产妇的二次剖腹产手术实施，临床应用价值相对较高。

[abstract]objective： to analyze the first cesarean section incision selection effects on secondary caesarean operation. Methods： a retrospective analysis in January 2016 to December 2016 in our hospital implementation of secondary cesarean section of 76 cases of maternal data， for the first time based on maternal incision cesarean section choice way， divided into crosscutting group （abdominal wall transverse mouth） and longitudinal cutting （longitudinal incision）， each group of 38 cases of maternal. Comparison of two groups of maternal open time， fetal childbirth time， intraoperative blood loss， and the patients postoperative abdominal adhesion degree contrast. Results：slitting open time of maternal and fetal childbirth time and intraoperative blood loss were lower than maternal crosscutting group， all the data contrast significant difference （p<0.05）. Slitting group had a significantly better than adhesion degree after ChanFuShu crosscutting group of mothers， compared statistically significant difference （p<0.05）.Conclusion：choose longitudinal incision cesarean section incision for the first time， the effect significantly better than choose abdominal wall transverse mouth， can shorten the open time of maternal and fetal childbirth time， reduce maternal intraoperative blood loss， and helpful to the second caesarean operation and implementation of the maternal relatively high clinical value.

剖腹產是高危产妇的主要分娩方式，随着近些年来人们生活观念的转变以及饮食结构的转变，剖腹产数量逐渐增加。剖腹产中切口方式的选择，会直接影响产妇二次剖腹产手术的效果，剖腹产会增加不良瘢痕形成的几率，产妇前置胎盘、胎盘粘连等问题的发生率也会显著增加，关系到产妇的生命健康。文章回顾性分析2016年1月到2016年12月在本院实施二次剖腹产的76例产妇资料，基于产妇首次剖腹产所选择切口方式，划分为横切组与纵切组。探究不同手术切口方式选择下，对产妇二次剖腹产手术的影响，现总结内容如下。

1 资料与方法

1.1 一般资料

回顾性分析2016年1月到2016年12月在本院实施二次剖腹产的76例产妇资料，产妇年龄在25岁到39岁之间，产妇平均年龄为（34.55±0.65）岁。孕周时间在36周到43周之间，平均孕周时间为（40.01±0.21）周。所有产妇均具有剖宫产指征，单纯子宫疤痕56例，臀位10例，胎儿窘迫6例，骨盆异常4例。……
