摘 要 目的：建立心脏重症监护病房（CCU）个体化分级药学监护档案表， 对患者进行全程药学监护。方法：结合CCU临床诊疗实际情况，回顾性搜集、整理和掌握患者的用药信息，根据实际情况定义监护等级，创建分级药学监护档案表。结果：分级药学监护档案表的制定大大提高临床药师的工作效率，患者的用药情况得到极大的规范和合理。结论：建立分级药学监护档案表，将协助医师用药做到个体化，努力实现“用药者健康利益最大化，药源性损害与成本最小化”。

Establishment of the grading pharmaceutical care file table for CCU patients*

ABSTRACT Objective： To establish a grading pharmaceutical care file table for CCU patients so that the whole process of pharmaceutical care can be implemented to the patients. Methods： Medication information about the patients was collected and retrospectively analyzed based on the actual situation of the clinical diagnosis and treatment at CCU， the monitoring level was defined based on the actual situation and a grading pharmaceutical care file table was finally established. Results： The working efficiency of the clinical pharmacists could be greatly improved and medication in the patients could be greatly standardized and trended to be more reasonable by the established table. Conclusion： The table will help physicians achieve individualized medication， and make an effort for drug users to achieve maximal health benefits and minimal drug-induced damage and cost.

据中国药学会科技开发中心的全国医药经济信息网统计数据表明，在2011—2015年间，全国各入网医院使用药品大类按金额排序，心血管系统药物用量一直处于领先。该类药物的应用十分广泛，也已经被人们接受和认可。而心脏重症监护病房（CCU）收治的患者具有病情危重，用药复杂等特点，部分患者罹患多种疾病，住院治疗用药品种繁多，合理用药形势严峻。临床药师药学监护（pharmaceutical care，PC）是提供直接的、负责的与药物有关的监护，目的是正确选择治疗药物，最大限度地降低药物不良反应及有害的药物相互作用的发生，改善患者生活质量，当前已成为药学服务新的工作模式[1]。 CCU临床药师参与全程药物治疗，可以协助医师提高治疗的安全性和有效性，体现了医院药学的核心价值。另外，部分医师对患者个体差异或者药物相互作用等关注度不够，用药不尽合理。……
