摘 要 目的：观察和评估单磷酸阿糖腺苷联合开喉剑喷雾剂治疗疱疹性咽峡炎患儿的疗效。方法：选取我院2013年1月— 2015年2月收治的100例疱疹性咽峡炎患儿，随机分为观察组和对照组各50例，对照组单纯使用单磷酸阿糖胞苷注射液治疗，观察组在对照组的基础上加用开喉剑喷雾剂进行雾化吸入治疗，对比分析两组患儿治疗的总有效率、临床症状的消失时间、CD4+、CD8+、CD4+/CD8+、白介素-10（IL-10）、g-干扰素（IFN-g） 的水平。结果：观察组患儿治疗的总有效率为94.00%，明显高于对照组的72.00% （P<0.01）；观察组患儿的咽痛、疱疹、溃疡和发烧等临床症状消失时间和IFN-g、CD8+水平均明显短或低于对照组 （P<0.01），而IL-10、CD4+、CD4+/CD8+水平则明显高于对照组 （P<0.01）；结论：单磷酸阿糖腺苷联合开喉剑喷雾剂治疗疱疹性咽峡炎患儿，能有效提高患儿的IL-10、CD4+、CD4+/CD8+水平，降低患儿的IFN-g、CD8+水平，提高患儿的免疫水平，改善其临床症状，效果明显优于单用单磷酸阿糖腺苷。

Effect of vidarabine monophosphate combined with Kaihoujian spray in the treatment of 50 cases of patients with herpangina*

ABSTRACT Objective： To observe and evaluate the effect of vidarabine monophosphate combined with kaihoujian spray in the treatment of patients with herpangina. Methods： One hundred cases of patients with herpangina were randomly selected and divided into an observation group and a control group with 50 cases each. The control group was treated with vidarabine monophosphate while the observation group with Kaihoujian spray besides vidarabine monophosphate. The total efficiency， the time for the disappearance of clinical symptoms and the levels of CD4+， CD8+， CD4+/CD8+， interleukin-10 （IL-10） and human g-interferon （IFN-g） were compared. Results： The total effective rate was significantly higher in the observation group than in the control group （94% vs 72%） （P<0.01）. The time for the disappearance of clinical symptoms such as sore throat， herpes， ulcer and fever and the levels of IFN-g and CD8+ were significantly shorter or lower in the observation group than in the control group（P<0.01） while the levels of IL-10， CD4+ and CD4+/CD8+ were significantly higher in the observation group than in the control group （P<0.01）. Conclusion： Vidarabine monophosphate combined with Kaihoujian spray for the treatment of patients with herpangina can effectively increase the levels of IL-10， CD4+ and CD4+/CD8+， reduce the levels of CD8+ and IFN-g， improve the immune level and clinical symptoms of patients， and its clinical efficacy is significantly better than that of single vidarabine monophosphate.

疱疹性咽峡炎是由柯萨奇A组病毒为主的病原体感染所引起的急性上呼吸道感染性疾病，传染性较强，临床上常给予抗病毒进行治疗[1]。苏志强等[2]通过对疱疹性咽峡炎患儿的免疫功能进行检测，研究发现疱疹性咽峡炎患儿的IL-10水平明显低于正常健康者，IFN-g水平明显高于正常健康者，说明疱疹性咽峡炎患儿均存在不同程度的免疫低下的情况，且免疫功能的低下与患儿的患病密切相关。临床上多采用单磷酸阿糖腺苷、开喉剑喷雾剂等治疗疱疹性咽峡炎[3-4]。但目前临床上关于单磷酸阿糖腺苷联合开喉剑喷雾剂对疱疹性咽峡炎患儿的疗效及免疫功能的影响的研究鲜见报道，我院将单磷酸阿糖腺苷与开喉剑喷雾剂联合治疗收治的疱疹性咽峡炎患儿，效果显著，现将结果报道如下。

1 资料与方法

1.1 一般资料
