【摘要】 目的 探讨低温等离子儿童扁桃体切除术后出血的原因及预防方法。方法 342例行低温等离子扁桃体切除术患儿中， 9例发生术后出血， 分析出血原因， 总结预防方法。结果 9例发生术后出血患儿中， 原发性出血1例， 继发性出血8例；扁桃体上极部出血2例， 扁桃体下极部出血7例；小量出血7例， 中量出血2例。经不同的方式处理后均痊愈出院。结论 低温等离子儿童扁桃体切除术后出血与周围组织损伤过多、术中止血不彻底、术后感染及术后饮食不当有关， 一旦发生中量出血， 须果断采取全身麻醉下手术止血。

【Abstract】 Objective To investigate causes and prevention measures of postoperative hemorrhage after low-temperature plasma children tonsillectomy. Methods Among 342 children receiving low-temperature plasma tonsillectomy， there were 9 cases with postoperative hemorrhage. Their hemorrhage causes were analyzed to summarize prevention measures. Results Among 9 cases with postoperative hemorrhage， there were 1 case with primary hemorrhage and 8 cases with secondary hemorrhage； 2 cases with tonsil upper pole hemorrhage and 7 cases with tonsil lower pole hemorrhage； 7 cases with small hemorrhage and 2 cases with median hemorrhage. All cases were cured had discharged by different treatment measures. Conclusion Postoperative hemorrhage after low-temperature plasma children tonsillectomy is related with too much surrounding tissue damage， incomplete intraoperative hemostasis， postoperative infection and inappropriate diet. Median hemorrhage requires immediate surgical hemostasis under general anesthesia.

扁桃体切除术以前多采用剥离法和挤切法， 为儿童常规手术方法。采取低温等离子刀手术出血明显减少， 损伤轻， 术后疼痛轻， 手术时间短， 继发性及原发性出血减小， 术后恢复快， 住院时间缩短。虽然是儿童常规手术， 但由于扁桃体反复感染致粘连严重、炎症控制不佳、伴有全身其他疾病或长期服用抗凝药物、术者操作粗暴， 术后出血是最常见的并发症。本文探讨低温等离子儿童扁桃体切除术后出血的原因及预防方法， 报告如下。

1 资料与方法

1. 1 一般资料 选取本科2010年1月～2015年8月收治的342例行低温等离子扁桃体切除术患儿， 男198例， 女144例， 年龄4～12岁， 均符合慢性扁桃体炎诊断标准。共9例发生术后出血。

1. 2 方法 所有患儿术前血常规及出凝血时间检查结果均在正常范围内。术后出血止血方法：7例出血患儿经双氧水棉球清除创面血凝块和污秽物， 棉球加压创面或钳夹等止血措施后出血逐渐停止， 同时给予输液、抗感染治疗。……
