蒙脱石散联合双歧三联活菌胶囊对小儿腹泻的疗效观察及机理探讨

目的探讨蒙脱石散联合双歧三联活菌胶囊治疗小儿腹泻的疗效及对VIP、NPY及5-HT的影响。方法本研究选取90例腹泻患儿，对照组（43例）给予蒙脱石散治疗，观察组（47例）采用蒙脱石散联合双歧三联活菌胶囊治疗。观察治疗后72 h内的疗效、腹泻症状消退时间、及对血浆中血管活性肠肽（vasoactive intestinal peptide，VIP），神经肽Y（neuropeptide Y，NPY）及肠黏膜5-羟色胺（5-hydroxytryptamine，5-HT）水平的影响以及探讨其疗效机理。结果治疗后72 h，观察组治疗有效率明显高于对照组（P＜0.05）；治疗后，观察组患儿腹泻症状消退时间总体上要明显短于对照组，与治疗前相比，治疗后观察组和对照组的VIP、5-HT水平均明显降低，NPY水平明显增高（P＜0.05），且观察组治疗后VIP、5-HT水平明显低于对照组，NPY水平明显高于对照组（P＜0.05）。结论综上所述，双歧杆菌三联活菌胶囊与蒙脱石散联用能迅速、有效改善小儿腹泻症状，有效调控VIP、NYP、5-HT的分泌和释放，减少胃肠道蠕动，减轻肠道炎症反应，从而改善腹泻症状，值得临床推广使用。

【Abstract】Objective Discuss the efficacy of triple viable bifidobacterium capsule combined with smecta in the treatment of children with diarrhea and their influence to VIP，NPY and 5-HT levels.Methods Selecting 90 sick children with diarrhea，from 6 months to 3 years，the control group（43 cases）was given smecta，the observation group（47 cases）was adopted smecta combined with triple viable bifidobacterium capsule.The efficacy of triple viable bifidobacterium capsule combined with smecta in the treatment of diarrhea was evaluated by recording VIP，NPY，5-HT after 72h therapy.Results After 72h therapy，the effective rate of the observation group was higher than that of the control group（P＜0.05）.After therapy，the time of each symptom of diarrhea subsided in the observation group was shorter than that of the control group（P＜0.05）.Compare with the values before therapy，the VIP，5-HT levels were decreased，while the NPY levels were increased in two groups after therapy（P＜0.05）.The levels of VIP and 5-HT in the observation group were lower while that of NPY was higher than those in the control group after therapy（P＜0.05）.Conclusion Above all，the triple viable bifidobacterium capsule combined with smecta could effectively improve the symptoms of diarrhea in children quickly，control the levels of VIP，NYP and 5-HT，reduce gastrointestinal motility and inflammation，thereby improving the symptoms of diarrhea，it is worth to clinical application.

小儿腹泻是临床上常见的一类儿科疾病，高发于6个月-3岁婴幼儿。小儿腹泻使得患儿体内水、电解质平衡紊乱，出现脱水现象，严重时影响婴幼儿的生命健康［1］。小儿腹泻的传统治疗以静脉补液为主，而该治疗周期较长，疗效低且延误治疗时机［2］。近年来临床多使用蒙脱石散或双歧杆菌三联活菌胶囊治疗小儿腹泻，疗效满意，为探讨蒙脱石散联合双歧杆菌三联活菌胶囊治疗小儿腹泻的疗效，本院儿科选取了90例腹泻患儿进行研究，现报道如下。……
