针刺联合井穴麦粒灸治疗急性脑卒中后痉挛性偏瘫临床观察*

目的 观察针刺联合井穴麦粒灸对急性脑卒中后痉挛性偏瘫患者的治疗效果。方法 将90例患者按随机数字表法分为对照组与研究组各45例。对照组实施常规治疗，研究组患者在常规治疗的基础上实施针刺联合井穴麦粒灸治疗，治疗结束后观察两组患者的临床症状改善情况以及治疗效果。结果 治疗前两组患者改良Ashworth痉挛评分（MAS评分）、肢体运动功能评分（FMA评分）、日常生活能力评分（ADL评分）等差异均无统计学意义（P＞0.05）。治疗后研究组MAS评分低于对照组（P＜0.05），研究组FMA、ADL评分均高于对照组（P＜0.05）；研究组治疗总有效率为88.89%，高于对照组的68.89%（P＜0.05）。结论 针刺联合井穴麦粒灸治疗能有效改善急性脑卒中后痉挛性偏瘫患者的症状。

【Abstract】Objective：To explore the effects of acupuncture combined with granule moxibustion in Jing acupoint on patients with spastic hemiplegia after acute stroke.Methods：90 cases of patients with spastic hemiplegia after acute stroke from February 2013 to June 2014 were selected as the research subjects，who were randomly divided into the control group and the study group，with 45 cases in each.The control group were given conventional therapy，and the study group were given acupuncture combined with granule moxibustion in Jing acupoint on the basis treatment of the control group.The improvement of clinical symptoms and therapeutic effects after treatment of the two groups were observed.Results：There were no significant differences in the scores of MAS，FMA and ADL between the two groups before treatment（P>0.05）.After treatment，MAS score of the study group was significantly lower than that of the control group（P<0.05），and FMA and ADL scores of the study group were significantly higher than those of the control group（P<0.05）.The total effective rate of the study group was 88.89%，higher than that of the control group 68.89%，and the difference was significant（P<0.05）.Conclusion：Acupuncture combined with granule moxibustion in Jing acupoint can effectively improve the clinical symptoms of patients with spastic hemiplegia after acute stroke，worthy of clinical promotion and application.

脑卒中又称中风，主要由急性脑血管疾病如脑梗死、脑栓塞、脑出血引起，脑卒中具有高死亡率、高复发率以及高致残率，是威胁人类健康的一大恶疾［1］。调查表明［2］，我国脑卒中的防病率高居世界第二，其中脑梗死发病率为0.11%，脑出血发病率为0.06%～0.08%。痉挛性偏瘫是脑卒中发生后最常见的并发症［3］，直接影响着患者的恢复情况以及生活质量。针灸治疗能显著提高肌力，明显降低肌张力，降低痉挛指数及改善痉挛肢体的运动模式［4］。笔……
