祛风通络散外敷配合康复指导对全膝关节置换术后关节功能的影响*

目的观察祛风通络散外敷配合术后康复指导对全膝关节置换术后关节功能的影响。方法 60例患者按随机数字表法分为对照组和干预组各30例。对照组给予常规术后康复指导，干预组在术后康复指导基础上辅以祛风通络散外敷，比较两组患者术后4周内各时间点患肢膝关节活动度，以及膝关节美国纽约特种外科医院（HSS）评分评价术后6周后的治疗优良率，并进行两组间对比。结果干预组膝关节屈伸活动范围恢复快于对照组，治疗后1周、2周、4周差异均具有统计学意义（均P<0.05）。4周疗程结束时干预组疗效优良率（膝关节HSS评分优良率）高于对照组（P<0.05）。结论全膝关节置换术后采用祛风通络散外敷配合术后康复指导有利于膝关节功能的康复。

【Abstract】Objective：To explore the effect of Qufeng tongluo decoction combined with rehabilitation instruction on knee joint function after total knee arthroplasty.Methods：60 patients with unilateral knee joint arthroplasty were randomly divided into the control group and the interventional group with 30 patients in each.The control group was given routine rehabilitation instruction after operation，while the interventional group was added Qufeng tongluo decoction.The differences of knee joint activity of each time points in 4 weeks and excellent rate of hospital for special surgery knee scores after 6 weeks between the two groups were compared.Results：The patients in the control group had a larger range of knee joint activities in 4 weeks.And their knee joint activities in 1 week after operation had statistical differences with the interventional group（P＜0.05）.The excellent rate of HSS scores of the interventional group were higher than those of the control group，and the difference was statistically significant（P＜0.05）.Conclusion：Qufeng tongluo decoction combined with rehabilitation instruction after total knee arthroplasty is conducive to the rehabilitation of knee joint function.

随着近年来骨关节病患者愈发增多，人工全膝关节置换术作为骨关节疾病终末期的治疗手段被广泛应用，而术后关节功能锻炼对预后及患者生活质量将产生极大影响，但临床较多患者因疼痛不敢早期进行康复训练或不知如何康复训练，而全膝关节置换术后1月内即会发生关节功能障碍［1］。因此，术后通过早期干预指导患者进行积极有效的康复指导十分必要。本研究选取行单侧全膝关节置换的60例患者，给予中药祛风通络散外敷结合术后康复指导，取得了较好的疗效。现报告如下。
