经尿道钬激光前列腺剜除术治疗良性前列腺增生40例效果观察

目的：观察经尿道钬激光前列腺剜除术治疗良性前列腺增生的安全性及疗效。方法：对我院2015年1月至2015 年7月收治的40例良性前列腺增生患者行经尿道钬激光前列腺剜除术，对术中、术后手术并发症进行统计，并对术前及术后3个月的最大尿流率（peak urinary flow rate，Qmax）、残余尿量（residual urine volume，PVR）、国际前列腺症状评分（International Prostate Symptom Score，IPSS）、生活质量评分（quality-of-life，QOL）等随访资料进行对比研究。结果：术后失访6例，纳入34例。患者术后3个月Qmax、PVR、IPSS及QOL较术前有明显改善（P＜0.01）。术中膀胱损伤1例，术后压力性尿失禁2例，尿道外口狭窄1例，所有患者未出现TUR综合征，无术中、术后严重出血需输血者。结论：该手术方式疗效良好、适应证广泛、安全性较高、风险较小，值得在基层医院推广应用。

［Abstract］Objective：To evaluate the safety and clinical effect of holmium laser enucleation of the prostate（HoLEP）for patients with benign prostatic hyperplasia（BPH）.Methods：A total of 40 patients with BPH from Jan 2015 to Jul 2015 were underwent HoLEP.Pre-and postoperative complication were recorded and the differences of the clinical parameters were also compared，including peak urinary flow rate（Qmax），residual urine volume（PVR），International Prostate Symptom Score（IPSS），quality-of-life （QOL）.Results：A total of 34 patients were included after operation and 6 patients lost to follow-up.Qmax，PVR，IPSS，QOL after three months postoperatively were improved significantly compared with that of preoperative（P＜0.01）.Complications included intraoperative bladder injury（1 case），postoperative stress incontinence（2 cases），urethral stricture（1 case）.No patients experienced the sign of TUR syndrome and blood transfusion for severe bleeding.Conclusion：HoLEP has the advantage of good therapeuticeffects，widescopeofapplication，bettersafetyperformance，smallerriskandit'sworthpopularizinginprimaryhospitals.

良性前列腺增生（benign prostatic hyperplasia，BPH）是泌尿外科的常见病和多发病，给老年男性患者带来较多痛苦，明显降低了其生活质量。经尿道钬激光前列腺剜除术（holmium laser enucleation of the prostate，HoLEP）是新型前列腺术式。到目前为止，HoLEP是唯一一个临床循证等级为1级的激光治疗BPH的方法，并同时得到美国泌尿外科协会和欧洲泌尿外科协会指南的认可［1-3］。其与开放前列腺切除术类似，如同“伸入尿道的示指”，刮开整个前列腺，在增生腺体和包膜之间“剥桔”式剜除，逆行将其从外科包膜上完整剥除，推入膀胱内，然后应用组织粉碎器将膀胱内的前列腺组织粉碎吸出。……
