中西医结合创新模式对糖调节受损患者强化管理的疗效评价

目的 评价中西医结合创新模式对糖调节受损（IGR）患者强化管理的效果。方法 收集于2014年1月—8月在北京市海淀区清河社区卫生服务中心门诊确诊为IGR患者60例，按抛硬币的方法随机分为干预组（30例）和对照组（30例），干预组予以中西医结合创新模式强化管理，为期1年；对照组患者行自主的常规门诊诊治。两组均在干预前后进行血液复查及问卷调查。结果 干预后干预组的体重指数、空腹血糖、餐后2 h血糖、糖化血红蛋白、血脂与对照组相比差异有统计学意义（P＜0.05）；在干预措施后，两组在糖尿病知识的认知（包括糖尿病基础知识、饮食治疗知识、运动治疗知识、药物治疗知识及血糖监测知识）和自我管理能力（包括饮食控制、每天坚持运动、主动监测血糖、随身携带援助卡及自我足部检查护理）差异有统计学意义（P＜0.05）；干预组患者3.3%（1/30）转为糖尿病，50%（15/30）的患者转为正常；对照组23.3%（7/30）转为糖尿病，20%（6/30）的患者转为正常，两组患者的转归差异有统计学意义（P＜0.05）。结论 采用中西医结合创新模式对IGR患者进行强化管理，可有效降低患者的血糖血脂水平，控制体重，减少糖尿病的发生。同时增强其对糖尿病的认识和自我管理能力。

［Abstract］Objective To evaluate the effect of intensive management on impaired of glucose regulation（IGR）patients in a Combination of Traditional Chinese and Western Medicine Team innovation model.Methods Totally sixty patients with impaired of glucose regulation（IGR）were involved in the study between 2014.1 and 2014.8 in Qinghe Community Health Service Center and were randomly assigned to the interventional group（n=30）and the control group（n=30）.Intervention group was under intensive management in a Combination of Traditional Chinese and Western Medicine Team for one year，and control group was given routine outpatient treatment.Both of them received blood reexamination and questionnaire investigation after one year.Results After intervention，compared with the control group，Body mass index,fasting blood glucose，postprandial 2h blood glucose,HbA1c and blood lipid of the intervention group were significantly different（P＜0.05），Knowledge of diabetes（including knowledge of diabetes,diet therapy,exercise therapy,knowledge of drug treatment and blood glucose monitoring）,and self-management skills（including diet control,daily exercise,active monitoring of blood sugar,carry a medical aid card and self-care）increased and the difference was statistically significant（P＜0.05）.The diabetes incidence of the intervention group was 3.3%（1/30），50%（15/30）of the patients returned to normal；The diabetes incidence of the control group was 23.3%（7/30），20%（6/30）of the patients returned to normal；the differences were statistically significant（P＜0.05）.Conclusion Intensive management of the Combination of Traditional Chinese and Western Medicine innovation model for impaired of glucose regulation（IGR）patients is an effective method to reduce blood glucose、lipid levels,control weight,and reduce the occurrence of diabetes.At the same time,strengthen the awareness of diabetes and self management ability.
