【摘要】 目的 探讨奥美沙坦酯联合硝苯地平对老年高血压伴微量蛋白尿的干预效果。方法 75例老年轻中度高血压伴微量蛋白尿患者， 随机分为A组（24例， 给予奥美沙坦酯联合硝苯地平治疗）、B组

（26例， 给予奥美沙坦酯治疗）、C组（25例， 给予硝苯地平治疗）。比较三组患者的血压及蛋白尿情况。结果 治疗后A组24 h尿白蛋白（ALB）（23.6±5.9）mg/L、血β2-微球蛋白（β2-MG）（2.6±1.9）mg/L、尿β2-MG（1.2±0.4）mg/L、尿免疫球蛋白G（IgG）（2.5±1.6）mg/L水平低于B组及C组， 差异具有统计学意义（P<0.05）。结论 奥美沙坦酯联合硝苯地平能显著降低老年轻中度高血压患者的微量蛋白尿水平， 方法简单可靠， 值得临床推广。

【Abstract】 Objective To investigate intervention effect by olmesartan medoxomil combined with nifedipine for senile moderate and severe hypertension complicated with microalbuminuria. Methods A total of 75 senile moderate and severe hypertension complicated with microalbuminuria patients were randomly divided into group A （24 cases， olmesartan medoxomil combined with nifedipine for treatment）， group B （26 cases， olmesartan medoxomil for treatment） and group C （25 cases， nifedipine for treatment）. Comparison was made on blood pressure and proteinuria in all three groups. Results After treatment， group A had all lower 24 h urinary albumin （ALB） as （23.6±5.9）mg/L， serum β2-microglobulin （β2-MG） as （2.6±1.9） mg/L， urine β2-MG as （1.2±0.4） mg/L and urine immunoglobulin G （IgG） as （2.5±1.6） mg/L than groups B and C. Their differences had statistical significance （P<0.05）. Conclusion Combination of olmesartan medoxomil and nifedipine can remarkably reduce microalbuminuria level in senile patients with moderate and severe hypertension. This method is easy and creditable， and it is worth clinical promotion.

高血压是能够引起脑卒中、冠心病、心力衰竭、肾衰竭等疾病的主要危险因素， 严重的危害着人类的身体健康， 已成为世界公共卫生的重大问题[1]。对高血压病的治疗仅仅单纯控制血压是不够科学的， 必须进行危险度分级。靶器官损害在高血压的危险分级中占有重要的地位， 肾脏的损伤尤为关键。近些年来的研究表明， 微量白蛋白尿是早期肾脏损害和内皮功能紊乱的重要标志之一[2]。采用尿微量白蛋白的检测成为诊断肾脏早期损害的一个灵敏性指标[3]。本研究探讨奥美沙坦酯联合硝苯地平控释片对老年轻中度高血压伴微量蛋白尿的疗效， 现报告如下。

1 资料与方法

1. 1 一般资料 选择2013年1月～2015年7月本院收治的老年轻中度高血压伴微量蛋白尿患者75例， 其中男42例， 女33例；
