摘 要 目的：探索糖尿病链式分层管理模式在社区应用后对患者自我管理水平以及血糖控制情况的影响。方法：2013年10月—2014年9月上海市徐汇区凌云社区卫生服务中心采用2型糖尿病链式分层管理模式对203例2型糖尿病患者进行管理。结果：管理1年后，患者的糖尿病自护行为得分、自我管理水平、空腹血糖、糖化血红蛋白控制水平均有明显提高（P<0.01）。结论：2型糖尿病链式分层管理模式是有效、积极的管理模式，值得在社区中应用并推广。

Preliminary evaluation of application of the hierarchical chain management mode of type 2 diabetes in a community

ABSTRACT Objective： To explore the effect of the hierarchical chain management mode of diabetes on self-management level and influence on blood glucose control in patients after it was applied in the community. Methods： From Oct. 2013 to Sept. 2014， 203 cases of type 2 diabetes were managed with the hierarchical chain management mode in Lingyun Community of Xuhui District， Shanghai. Results： After one year management， the score of the self-care behavior， self-management level， fasting blood glucose and glycosylated hemoglobin control levels of the patients were significantly increased（ P<0.01） . Conclusion： The hierarchical chain management mode is an effective and positive mode， and it is worthy of application in the community.

目前，随着经济的迅速发展和生活行为方式的改变，我国糖尿病的发病率正以惊人的速度增长，已达9 240万[1]，已成为全球糖尿病第一大国。探索符合中国国情的有效、合理的糖尿病管理模式对于糖尿病的防治有着重要意义。依托社区开展防治工作是预防控制慢性病的最有效措施。但社区还存在医疗条件有限、卫生人员专业知识局限的问题，而专科医师的专业知识水平高，可以针对患者的具体情况提供合理有效的治疗方案，但在提供后续的管理方面有所欠缺，对患者的遵医嘱行为和血糖控制情况不能全面了解。分层链式管理模式是一种将社区一、二级医院的资源有效整合、医患结合的糖尿病管理模式，本研究评估在社区应用该管理模式管理2型糖尿病患者的效果，探讨该模式在社区应用后对患者自我管理行为以及血糖控制情况的影响。

1 对象与方法

1.1 研究对象
