超声三维斑点追踪技术对肥胖和非肥胖2型糖尿病患者心脏功能的评价

目的探讨对超声三维斑点追踪技术对肥胖和非肥胖2型糖尿病患者心脏功能的评价。方法 随机择取在2015 年4—10月半年期间于该院就诊的患有糖尿病的患者100例，将100例患者按照是否合并全身性的肥胖症状的方式分成两组，令两组人数均为50例患者。除此之外，择取50例健康志愿者设为健康组。对上述3组患者进行超声检测。结果在糖尿病患者当中，存在全身性肥胖症患者无论是在体质指数、舒张压以及腰围、左心室内径、心室内壁厚度上均明显高于另外的两组（P＜0.05）；另外，在空腹血糖值以及收缩压上也明显地高于健康组（P＜0.05）；但是在2D-上，则显著地比另外两组要低。相对的，没有合并发作全身性肥胖的患者，则仅仅在心室壁的厚度值上明显地比对照组要高（P＜0.05）。结论糖尿病患者如果合并肥胖，可能会导致收缩功能受损更加严重，三维斑点追踪手段能够非常敏感的找到常规性超声心动图比较难以检测出的细微性变化。

［Abstract］Objective To investigate the evaluation of the cardiac function in obese and non obese patients with type 2 diabetes mellitus by three dimensional speckle tracking technique.Methods Randomly selects in April 2015 to in October of the same year during first half of the year in our hospital with diabetic patients with 100，100 patients with systemic symptoms of obesity divided into two groups according to the，make two groups of people were 50 patients.In addition，we take 50 healthy volunteers as healthy group.Ultrasonic testing was performed on the three groups of patients.Results In patients with diabetes，there are systemic obesity patients both in body mass index（BMI），diastolic blood pressure and waist circumference，internal diameter of left ventricle，ventricular wall thickness were significantly higher than the other two groups are significant（P＜0.05）；in addition，the fasting blood glucose and systolic blood pressure are significantly higher than healthy group are significantly（P＜0.05）；but in 2D，significantly more than the other two groups to low.In contrast，patients who did not have generalized obesity were significantly higher than those in the control group only on the thickness of the ventricular wall（there was a significant P＜0.05）.Conclusion Patients with diabetes if combined with obesity may leads to impaired contractile function is more serious，3D speckle tracking methods can be very sensitive to find regular ultrasound cardiogram is difficult to detect subtle changes.
