斑点追踪成像技术评价不同程度心室重构高血压患者的心房功能

【摘要】目的 探讨斑点追踪成像技术评价不同程度心室重构高血压患者的心房功能。方法 选取2013年9月～2014年6月我院确诊的原发性高血压患者122例。依据Ganan提出的按照左室质量（LVMI）和相对左室壁厚度（RWT），将原发性高血压左室重构分为四组，即正常构型（NG组）、向心性重构组（CR组）、向心性肥厚组（CH组）以及离心性肥厚组（EH组），另收集30例患者为正常对照组。以斑点追踪成像技术测定左心房功能。结果 正常结构组患者和向心性重构组患者的左心室心尖部旋转角度峰值（PAR）高于向心性肥厚组和离心性肥厚组，差异有统计学意义（P＜0.05）。结论 斑点追踪成像技术评价不同程度心室重构高血压患者的心房功能效果较好。

Objective To evaluate the atrial function in hypertensions with different types of ventricular remodeling by speckle tracking imaging. Methods 122cases of primary hypertension diagnosed in our hospital from September 2013 to June 2014 were selected. According to the left ventricular mass（LVMI）and relative left ventricular wall（RWT）which Ganan proposed，hypertensions who with left ventricular remodeling were divided into four groups，normal control group（group NG），concentric remodeling group（group CR），concentric hypertrophy group （group CH），eccentric hypertrophy group（group EH）and 30 cases of normal. Speckle tracking imaging was used to detect left atrial function. Results Peak apical rotation（PAR）of left ventricular in normal control group and concentric remodeling group was significantly higher than concentric and eccentric hypertrophy groups，the difference was statistically significant（P＜0.05）. Conclusion Speckle tracking imaging was effective in evaluation of the different types of ventricular remodeling in hyperteinsions.

高血压是临床常见病、多发病，及时有效的诊治对患者有着极为重要的意义［1］。对高血压患者的诊治过程中，对其实施及时有效的心室重构的诊断极为重要，在本次研究中，运用斑点追踪成像技术（speckle tracking imaging，STI）评价不同程度心室重构高血压患者的心房功能［2］，现报道如下。

1　资料与方法

1.1研究对象

研究对象为2013年9月～2014年6月我院确诊的原发性高血压患者122例，其中男81例，女41例，年龄49～78岁，平均（59.65±6.54）岁。诊断标准参照2012年《中国高血压防治指南》，收缩压≥140 mm Hg和（或）舒张压≥90 mm Hg，经询问病史，体格检查，心电图及超声心动图检查，排除瓣膜性心脏病、心肌梗死、心肌病等疾病。……
