[摘要] 目的 研究孕妇血清P-亮氨酸氨基肽酶（LAP）检测在异常妊娠中的应用价值，为临床异常妊娠的检测提供参考。 方法 选择我院2014年2月～2015年4月收治的妊娠产妇作为本次研究的对象，包括异常妊娠及正常妊娠，比较其血清LAP、碱性磷酸酶（ALP）、丙氨酸氨基转移酶（ALT）活性。 结果 早期妊娠组中，异位妊娠产妇的LAP活性低于正常妊娠者，差异有统计学意义（P<0.05），但两者ALP和ALT的活性差异无统计学意义（P>0.05）；中期妊娠组中，先兆流产和正常妊娠产妇的LAP、ALP、ALT活性比较差异无统计学意义（P>0.05）；晚期妊娠组中，正常妊娠和多胎妊娠、胆内胆汁淤积症的LAP、ALP、ALT活性比较，差异有统计学意义（P<0.05）。 结论 孕妇血清P-LAP检测在异常妊娠中具有积极的价值，可有效监测妊娠过程中是否存在异常情况。

[Abstract] Objective To study the clinical value of serum P-leucine aminopeptidase （LAP） detection in abnormal pregnancy and to provide reference opinions for the detection of abnormal pregnancy. Methods Pregnant parturient from February 2014 to April 2015 of our hospital were selected as experimental objective，including normal pregnant and abnormal pregnancy parturient.The activity of serum LAP，alkaline phosphatase （ALP） and alanine aminotransferase （ALT） in two groups were compared. Results In early pregnancy，the activity of LAP in ectopic pregnancy group was lower than that of normal pregnancy （P<0.05），but the activity of ALP and ALT between two groups had no obvious difference （P>0.05）.In mid pregnancy，the LAP，ALP and ALT between threatened abortion group and normal pregnancy group had no obvious difference （P>0.05）.In third pregnancy，the activity of LAP，ALP and ALT between normal pregnancy group and multiple pregnancy group，intrahepatic cholestasis had obvious difference （P<0.05）. Conclusion Pregnant serum P-LAP detection has positive determination value in abnormal pregnancy，which can effectively monitor whether there is any abnormal situation during pregnancy.

胎盘亮氨酸氨基肽酶（LAP）是人体中能够降解多种多肽的降解催产素，临床上亦称之为催产素酶的膜结合型外肽酶[1]，如血管紧张素和抗利尿激素等。妊娠期间，产妇为了适应胎儿的生长发育，孕妇体内的激素和酶类也会出现相应的生理变化，其LAP活性在一定程度上可以较好地反映产妇的妊娠情况[2]。本文为进一步探讨孕妇血清P-LAP检测在异常妊娠中的临床价值，选择我院收治的异常妊娠产妇和正常妊娠产妇作为研究对象进行研究。

1 资料与方法

1.1 一般资料

选择我院2014年2月～2015年4月收治的妊娠产妇作为本次研究的对象，按照妊娠时间分为早期妊娠组、中期妊娠组、晚期妊娠组，每组均有异常妊娠产妇及正常妊娠产妇，其中早期妊娠组有25例异位妊娠、25例正常妊娠产妇；中期……
