【摘要】 目的 探讨分析改良术式结合中药治疗肩锁关节脱位的临床疗效。方法 90例Tossy Ⅲ型肩锁关节脱位患者， 随机分成A、B、C三组， 各30例。A组行改良术式结合中药治疗， B组行喙锁韧带重建治疗， C组行锁骨钩钢板固定治疗。比较三组疗效。结果 A组优良率明显高于B、C组（P<0.05）；三组复发情况比较差异无统计学意义（P>0.05）。结论 改良术式结合中药治疗肩锁关节脱位疗效确切， 可避免二次手术， 费用低廉。

【Abstract】 Objective To investigate and analyze clinical effect by modified operation combined with traditional Chinese medicine in the treatment of acromioclavicular dislocation. Methods A total of 90 patients with Tossy Ⅲ acromioclavicular dislocation were randomly divided into groups A， B and C， with 30 cases in each group. Group A received modified operation combined with traditional Chinese medicine， group B received coracoclavicular ligament reconstruction， and group C received clavicle hook plate fixation for treatment. Curative effects of the three groups were observed. Results Group A had obviously higher good rate than groups B and C （P<0.05）. The difference of recurrence condition had no statistical significance in the three groups （P>0.05）. Conclusion Modified operation combined with traditional Chinese medicine provides precise effect for shoulder joint function in treating acromioclavicular dislocation， and it avoids re-operation with low cost.

1 资料与方法

1. 1 一般资料 选取2014年1月～2016年1月抚顺市中医院骨科收治90例Tossy Ⅲ型肩锁关节脱位患者， 随机分为 A、B、C三组， 各30例。A组男23例， 女7例， 平均年龄43岁；B组男18例， 女12例， 平均年龄39岁；C组男16例， 女14例， 平均年龄36岁。三组一般资料比较差异无统计学意义（P>0.05）， 具有可比性。

1. 2 治疗方法 A组行改良术式结合中药治疗：手术均在全身麻醉下， 取肩锁关节部U行切口， 显露脱位的肩锁关节， 改良术式及喙锁韧带重建均取自体腘绳肌腱， 改良术式是在喙锁韧带重建的基础上， 将从锁骨骨隧道上拉出并固定完毕的自体肌腱移植物向外侧肩锁关节囊部反折， 同时用自体肌腱移植物与断裂的肩锁关节囊、肩锁韧带作编织缝合， 修复并加强损伤的肩锁关节囊、韧带。术后肩关节不行外固定， 引流管拔除后即行肩关节的被动练功， 2周后给予中药熏洗， 方法：将煮好的熏洗药液放置盆内， 约500 ml， 暴露肩关节于盆上， 让热气熏蒸患部， 用毛巾浸药液热敷患处， 肩关节发热后开始练功， 持续熏洗1 h/次， 2次/d， 熏洗方药如下：伸筋草15 g、透骨草15 g、延胡索9 g、当归10 g、姜黄9 g、川牛膝10 g等。……
